INTROUCTION
Change is the permanent and change in the regulation where there is for benefit which is
highly required for better health of patients and for the health organisation. There are different
advantages of change which is for better. To understand the importance of change which is there
in health and social care is highly required. Aim of this report is to understand the importance of
change which can lead to high benefit for patients along with regulation of health care
department. In this report, there is discussion about the reason for change and which type of
change should be there which includes external environment analysis, change on patient level
and organisation level, leadership role etc. There is discussion about the strategic plan and its
implementation which includes 8 point approach which is there to develop a change
management. There is also analysis of benefits in relation to sustainability of change (Cutler,
2019).
MAIN BODY
Background information about two patients
Patient 1 is 22 years old who is suffering from known personality disorder along with
anxiety illness. Patient has presented to A&E 590 times. There is involvement of multiple
agencies which are there involved in treatment. There can be seen that there are is change in
appointments which is there in after April when there change has been introduced.
Patient 2 is 66 year old who is suffering from dementia and have poor social
circumstances. Patient have a daughter who is having addiction issues. In relation to this, there
are different concern which are there regarding the safety of patient. General practitioners needs
to get more information regarding the patient and may be chances to share those health related
information with daughter for ethical concern (Rocque and et. al., 2016).
Analysing the reason of change in vulnerable patient care and what need to change
Patient 1- Regarding with patient there is need for the change which is there in the A&E
visit. This can be necessary to bring change in the visit for the A&E. It can be analysed that there
is no any beneficial change which is there in positive in nature and have benefit for patients.
There is high need to for the reduction in the visit of A&E. As per the analysis and medical
history, it can be seen that there is no any type of better response can be seen in patient. Patient is
suffering from anxiety disorder which is there need for better care and regular treatment. There is

need to change in the treatment process which includes different new procedures which can have
better response. Change in the treatment procedure can have better effect which is there also help
to achieve and learn the new way to treat such type of patient. This can help in better way to
patient (Schellinger, 2018).
Patient 2- There can be seen that there is need to reduce the visit which is there related
with A&E. Analysis can help to support this statement which is there regarding better and new
treatment procedure. There is no any better condition of patients condition and there is need to
do some changes in the treatment procedures which are there able to help the patient in better
way. There is no one who can care for the patients because due to having daughter who is having
the addiction issues can leads to no advantage of her presence in relation to providing care to the
patients. Patient is suffering from the dementia problem and due to not having the social well-
being there is no one who can help to provide better care and treatment. So, in relation to this
there is need to use some different treatment procedure which can help to get better condition of
patient. This can be done when there is reduction in the visit of A&E.
Improved patient safety in social environment
Patient safety concerns with the reducing and preventing the risk which are here
associated with the patient due to having their disease problem and there are different issues
which are there can be out in normal world where every one is busy in their life. There is need to
provide better care to the patient who are there can have better safety from any issues or
unfortunate incident which can be dangerous for the patient. In relation to this, there is high need
of care to the patient which should be there for better treatment and having better life which can
only be done by getting proper care and medication which is necessary for their better health.
Social environment is the one which is there able to help and provide the better environment
which can help the patient for better health and along with this, this social environment can help
to decrease the problem and create the positive impact on the patient health. Social environment
is the one which can help to get better improvement to the patient and there can have better
interaction with different people and being social can help to give some relief to the patient. This
can help patient in better health which is there due to having better life style. There is highly
need to take care about the patient who there out in society and having social interaction which is
regarding their safety concern (Pinkert, 2018). There can be any chance when patient is there can
harm himself or to the other person which is due to their any side effect of their illness or any

other cause. There is high need to monitor the patient who are there in the social environment
where they are interacting with different people either to other patient or any person who are
there with their different moods. During the social interaction there is high need for the safety of
patient who should need to follow all the medical procedures along with medication which are
there for the better health and physiological health of patient. Care taker or the family who are
there for the care of the patient should ensure about their safety and need to have continue check-
up without regular informing them to not to talk with other or any type of behaviours which are
there not good as per the interaction of the patients with society people. Safety of patients is
highly necessary which is regarding the better health and to provide better lifestyle.
Improved quality of care and well-being of vulnerable patients
There is high need to understand the patient and to fulfil their necessary requirement.
There are different steps which can help to improve the quality of care and their well-being of
vulnerable patients. In many there are some of the quality care which can be improved by some
applying some examples like having the pharmacist medication therapy management, optimizing
sepsis care which can improve the early recognition and their outcomes along with this there is
operational techniques, data driven approach are those which can help to improve the quality of
care which is there also able to improve the well-being of vulnerable patients. Pharmacist
medication therapy management is the one which is there able to help to reduce the cost of
treatment which is there in the hospital. In this, there having the improvement in the care along
with this there is also help in the financial cost which are there too much higher in the hospital
can be saved (Deeken, 2019). In this, there is less cost of management and due to this, there is
high quality of care along with there are improvement in the patients care. Optimizing sepsis is
the one in which is helpful in early recognition or any complications. There are different type of
changes in the operational process which can leads to improvement in the quality care which is
there due to using the different new operational techniques which have better effect on the
patients health and could have better outcome. Improvement in the quality of care is very
necessary for both of the patient which is there in relation to the better health and improvement
in the health condition.
At the organisation level in relation to provide better care, there is need to improve the
efficiency of partnership, there is also need to reduce the cost which can help to provide better
care to the patient. There is need to avoid the unnecessary waste of resources which is there in

et. al., 2018).
PESTLE analysis
Political- This is the first factor which is there have the high effect on the procedure of
health care. In this, there is need for the approval of new changes which is there for providing
better care to the patient.
Economic- This is the second factor which is there dependent on the economy in relation
to implement new ideas, there is need for high finance where there are different steps which
require financial cost which includes establishment of equipment, machinery, providing training
and maintenance of the equipment etc.
Social- in this, due to change in the medication and treatment procedure can help to get
better care of the patient which is related with social care.
Technological- This is the one which is high effective in nature and have effect on the
procedure of treatment which is base on the new changes (Bammann, 2020).
Models, theories and concepts of process of change
There are different theories which are there can help to understand and leads to change
process which is there in health and social care. There are different models like Lewin's change
management model, McKinsey 7-S Model, Nudge theory, Kotter's theory etc. There are some of
the models which are there for the change are discussed further.
Lewin's change model
There are three different phase model which are like unfreeze, change, refreeze.
Unfreeze- This is the one in which there is determination of what is there need for
change. There is need to ensure about the strong support by management, there is also create the
need which is for change along with this, there should manage and understand concern and
different doubt which are there during change.
Change- in this, there is often communication is there and able to empower action. Along
with this, there is also involvement of people in this process. There is change in the organisation
which is to provide the better health and social care.
Refreeze- in this, there is refreezing of the changes which is there in culture. There is
also development of different ways through which changes should be stable and sustain. This is

This model was developed by Robert Waterman and Tom Peters. This model refers to the
powerful tool which is there to access the internal situation of the organisation. In this, there is
need to highlight the changes which is there need to made. There are hard element and soft
element which includes strategy, structure, systems, shared values, style, staff and skills.
In this, there is involvement of seven aspect of company which is there need for change
and able to provide better health and social care to their patients (Yushak, 2018).
Hard elements- These elements are the one which are direct control of the management and
need to easily be identified and defined.
Strategy- This refers to the action plan which is there able to provide the competitive
advantages to the organisation.
Structure- This refers to the structure of organisation which is there able to better handle
each and every function within an organisation and are the one who are responsible for each and
every function within organisation.
System- this includes the day to day activities in which each and every staff member are
involved and must get ensure about the completion of task which has been assigned. In relation
to change in health and social care there is need to have the proper information which is there
can have the better information regarding regarding change.
Soft elements- These elements are less tangible along with difficult in nature and are mostly

organisational skills and project management. They are also possess some advanced skills like

manger are the one who utilizes different tolls which are there used to perform their job such
tools like analytic tools, MS office application, internet along with transition management
software (Evans, 2018).
Situational variables- These are the factors in the business environment which are there
unintentionally affect the situation of the business and impact the result of the change which is
there during the change in organisation for the better health and social care of patients. These

health care professional and their training in better manner can help the health professionals to

How to implement the plan
This can be implemented by involving each and every employee and function of
management who are there can help to accept the change and to learn the things which are there
required in sense to implement the change in better way. There is need of transition manger who
is there can play a vital role in the change. There is need to plan each and every function in such
a manner that can have better result in practical manner.
Communication change
There are four features of communication networks like role, content, directionality and
channel. In this there is need to inform the staff members which is regarding the need for change,
what is going to happen, what should be done according to requirement etc.
Refining of plan
This is the one which is there need to reflect the idea which is unique in nature in which
there should be for the patients. In this, there is need to refine the plan which is there able to get
the proper information which is regarding the plan along with this, there is need to inform each
and every person who is there working as a staff member or the health professional. This can
help to get the better implementation of plan and there could be better support to the patient. This
