The following report is prepared in order to understand why ity is important to bring
change in the heath and scare facilities in order to improve the conditions of the disabled
patience. Denise is an adult home care who provide health care to the physically disabled or
mentally less developed people (Henderson Beach and Finkelstein 2011). They provide them
with the services like washing and cleaning and take care facilities so that they can continue with
their lives with less challenges. In order to run such type of organisations there is a need to take a
prior licence from the government and the challenge which they face is people resist to shift from
there resident to these home care services provider.

Community’s misconceptions- people suffering from the disabilities finds it difficult to live
comfortable in the society where other people are not as same as they are. They underestimate
themselves and never try to live with the full confidence. The society also contribute in this by
making them feel different from the others by continuously making them realise about their
disability. This brings feeling of ignorance and discrimination among them. In the case where the
child or the parent is disabled for the life time starts becoming a burden to their family members
after a certain time period therefore it is better if they are shifted to the care taking authorities on
time (Miller and Rollnick,2012).
1.School related challenges- it is difficult for the physically challenged students to get
education. When they are ta home although they can learn from their family members but the
education which is provided at school are of different level. When they are shifted to the care
homes they are benefited with more education as there arrangements are done for all the children
so that they are not deprived of from their right of being educated.
2.Balanced diet-People who are challenged in any way of medical problem need to be
provided with extra nutrition and balanced diet. At homes they do not starve but also they cannot
maintain the required diet chart which is very necessary for them. When they are under the
supervision of doctors all the time they will have only that which is actually good for them
(Ertmer. and Ottenbreit-Leftwich,2010). Therefore this will help them in getting the balance diet.

has positive outcomes it is difficult in managing the change at its initial stages. When a
difference was brought in the health and care supervise by the management of Denise many
challenges were faced by the employees, the subject matter and the family members of the
challenged people. These challenges can be put under different heads like:

support living from there residential homes the main challenge was off to convince them to
support the idea. They find it difficult and risky to shift in a new place where there will be all
new people and there will be no member from there family. It was difficult to win over there

of them then these together works all about generating creating and up bring the “climate for
change “, further to add up next few steps focus on engaging and enabling the organisation ,and
last remaining steps tries to focus on implementation and sustaining changes(Eppig Blake,

“If you wish to make a future you must make it more probable” then there comes the third step
that is create and develop a good vision for change: the need of clear vision is to make
people understand that why are doing this thing what the need of change, when start
thinking about changes it basically new ideas comes to keep them all to make your vision
clear .What you need to do is create a plan for your vision try to summaries your plan to
others for clear vision.
Now under engaging and enabling the organisation , few steps comes that are communicate
the vision and next is remove obstacles and the further step is to create short term wins ,the
communication is not just data transfer but something more remove the obstacle which don’t
let you bring changes and start celebrating small wins . Now under implementing and
sustainable change comes last steps that are built on the change and transom it into proper
things, after ever victory analyse what is right and what is required to improve for good in
future
3.2 Explain how changes in health and social care are planned:
After the success of Dennis it was observed that it is good to bring change in the other
health and social care providing units. For this effective communication between the
management of the Dennis and the other institutes was done so that they can interact and realise
how it is better to have the independent living system in the country so that they also contribute
in this. Also in order to bring the change in their own unit the staff is well communicated about
the objectives and also there vision is made clear as how they can achieve those goals. For this a
set hierarchy is followed for the flow of information about the change and there effect which will
be observed in the future.
3.3 Assess how to monitor recent changes in health and social care services.
Whenever any change is brought it is necessary to monitor it on a continuous basis in
order to check its efficiency and effectiveness in the organisation. For this control needs to be
done on the different activities so that if any change is required it can be brought at the same
time. Also this help in reducing the time wastage of making the corrections in future (Amundsen
Berglund and Westskog,2010). The change can also be monitor through observing the consumer
and employee satisfaction as if they are satisfied than we can say that the change is effectively
implemented in the organisation. Their satisfaction is must as if even if one of them is not
satisfied by the change brought than it cannot continue for a long period of time. Therefore
