Evidence-based practice (EBP)
There is acknowledgement that older adults with heart failure do not adhere to
medication. This may be as a result of different factors. However, translational model has been
found to be very effective in addressing this medical issue by using the ace star model of
knowledge transformation. According to Stevens (2013), the ace star model of knowledge
transformation comprehensively translates evidence into practice. In this situation, this model is
very effective because it organizes both the new and old care improvement concepts into a whole
and also provides a framework in which the processes and approaches of evidence-based practice
can be organized. To ensure that the older patients adhere to their medication, the
multidisciplinary team must discuss the practice (EBP) changes, evidences which support the
changes and the outcomes. The EBP must clearly describe how the knowledge is translated into
practice by using the ace star model to highlight the important concepts. First, the team must
evaluate the data presented and the essence of further action in regard to the non-adherence to
medical in older adults with heart failure. The team will then present evidence of discovery as
well as summary to conclude the statement (Curtis, Fry, Shaban, & Considine, 2017). Further,
the team will be required to translate and integrate the process of change and the improvement
activities which are related to the implementation. The team will then evaluate the outcomes of
the changes by highlighting the improvements made in relation to the EBP change.
