INTRODUCTION
Social needs refer to the need to have relationships with others once the physiological and
safety needs have been fulfilled. Maslow considered the social stage an important part of
psychological development because our relationships with others help reduce emotional concerns
such as depression or anxiety.
This report will cover the 3 section and each section has been define the specific cases
study of the 3 different person. This is an case study of Patrick Green who is suffering from the
HIV positive, Brands who has an mental problem and her age is also 58 and last case study is
about the Maria who is 34 years old living with her mother, and she is an class teacher of the
primary school and her mother is having Dementia and because of that she have to care about
her mother and she cannot sleep properly.

Agency plays important role in the individuals' life because some of the people need the
support from the some medical experts who can concern the problem wit them and provide the
proper plain of the care and their improvement in the health (Whitlatch and Orsulic-Jeras.,
2018 ). There are many people who does not care about their health themselves and they need
help from the medical professionals who can provide them proper advice and also take care of
them. This agencies and social care workers provide the helps to those people who are suffering
from the disease, mental problem and other things.
There are many different different organization who provide the help to the people
who are facing the problems in their life and they need proper consistency and care in their
life. NHS is one of them who is one of the most social care providing institution. They are
running their activities in all over the UK and their health provider facility is professional, and
they are expert in this field. The UK has a government-sponsored universal healthcare system
called the National Health Service (NHS). The NHS consists of a series of publicly funded health
care systems in the UK. It includes the National Health Services (England), NHS Scotland, NHS
Wales and Health and Social Care in Northern Ireland.
Role of organizations responsibility for commissioning healthcare services
Clinical commissioning Group which is known as CCGs group are the NHS
organizational and this are set up by according to the Health and social care Act 2012 to

organize the delivery of rage NHS services in the England . This institution provider the health
and well being advice to the social group in the England and their aim is to make health of the
social better and provide the social care top the needy people (Castrucci and Auerbach., 2019).
The members of the the CCGS find the people or contact the people who need the care and
help from the social care institution. When they find the people and any person connect with
them then they provide the consistency with the expert and their members provide the proper
care of that person.
Role of organizational for the commissioning social care services ( local authorities )
Local health care authorities like small health care providers who provide their
services in the local areas of the England. They are having very less members but they give
their best to provide person possible help and provide the better consistency to the people who
are suffering from the problems. They give the suggestion about the better treatment and
profession. Arrangement of meeting where the expert and the person who need help can talk
about the problems and take the solution for the problems. They can take the treatment by
getting the better advice from the experts.
Role of Bodies responsible for the integrating the health and social care
HWB's Health and Well being Boards (HWBs) are a statutory forum where political,
clinical, professional and community leaders from across the care and health system come
together to improve the health and well being of their local population and reduce health
inequalities. portfolio-holders/lead members for public health.
This is the duty of the HWBs to ensure the health of the local community and find the -
people who are suffering from the problems. In the case of Patrick are not getting the proper
healthy care and the support from the HWBs board because there is no treatment of the This
disease and they cannot the disease of the both person Patrick and James. They give the advice
which can reduce the effect which is about the using the protection and Terence Higgiens trust
also offers them the offline consultancy for the advice, and they can live the life properly.
This was also an mistake of the Patrick because he was known that James is suffering from this
problem then also he makes the relationship with the James and this result, Patrick get
infected (Meagher and et.al., 2019). They also get the treatment plan from the G.P and get the
advice to use the Condom when they make the relationship with each other.
Role of assessment and Eligibility framework

common assessment framework
The common assessment framework is the appropriate framework to find out the
children have some addition needs , assess need and the strengths and the agencies provide the
support to those children and help them to fulfill their needs. They work on the particular age
group of the Children. Their main focus is to provide help to children who need helps and
their aim is to provide them health, satisfy and economic well being. The CAF is helping the
children who are support from the disease, children who are not having parents and mentally
and physically dabbled children.
The national Eligibility Criteria ( care Act 2014)
the National eligibility criteria comes under the health care act 2014 and this
framework helps t determine whether any of the assessed care and support needs meet the
threshold for the support from the local; Authority. There are some articles in which people
can apply for the treatment, and they will get the professional treatment from the health care
professional without paying fees.
There are some particular areas in which people can get the treatment and according to
the each case study Patrick cannot apply for this because there is no treatment of HIV so the
Patrick is not liable for it but Brand can apply for the this care act because their treatment is
possible and Maria mother can also apply for it because her treatment also can be possible, and
they eligible for the criteria.
National Framework for NHS
A National Service Framework (NSF) was any of several policies set by the National
Health Service (NHS) in England to define standards of care for major medical issues such as
cancer, coronary heart disease, chronic obstructive pulmonary disease, diabetes, kidney disease,
long-term conditions, mental health, old age (Goetz., 2018). Their main aim is to look for the
health of the community and provide the treatment to the needy people.
Yes, Brands is eligible for the National Service Framework ( NHS ) because she is
suffering from the mental illness and they provide the treatment related to the mental health of
the person

The multidisciplinary team is the proper group of the health care worker, and they can
be related to the professional, social workers, Psychiatrists etc. each of the member in this team
provide social treatment to the patents who are expert in their category.
The multidisciplinary team helps the people through connecting them through their
agencies and give the advice to them about their treatment The multidisciplinary team includes
doctors, nurses, allied health professionals and pharmacists. Other members may be co- opted
into the ward round as appropriate to the patient group (Moghaddam and et.al., 2016). All
members of the team should have the opportunity to actively interact as part of the ward-round
process.
Role and responsibility
There responsibility is to provide the care and treatment to the patent and support them
to meet their needs and their demand. For example - if any person is having a hart problems a
then they will need the professional who heat doctors. The doctor from the multidisciplinary
team will meet with the patent and ask him about the exact problems, and they will also treat in
the hospital and provide them advise how he can care about their hart. Another example
from the multidisciplinary team is the one of the member who is known as Psychiatrists, and
they will responsible to provide the treatment to the person who are suffering from the mental
illness, and they will consult with the patent and ask about their problems and provide the
strategies and proper plan to overcome from the mental problems which they are facing in
their life. They will get the proper health care from the Psychiatrists, and they will treat them
properly through their advice.
Sexual Health Advisers roles and responsibility
Sexual health advisers are the spaciest in the problem which are related to the sexual
relationship ad the problems they provide assessment, basic health advice and information to
patients, supporting them to access the appropriate care, including advice to manage their
symptoms at home. They give advice and treatment to the patents who are facing the problems
which is about their sexual life.
SHO is the healthcare professional who work for the treatment of the disease which are
connected with the sexual life of the person, and they are facing any problems or the infarction

on their sex organs. People need advice from them because the sexual problems can affect the
physical health of the people and some of the infection are dangerous for the person so people
t should take the advice before doing any sexual activities.
SHA meets Patrick needs
Simon was the SHA of the Patrick and he has been assured by the Simon that all
information will be remain confidential and the it will not affect their status and the center.
And their SHA also advice them to take the appointment in his office including the James
because he is also om the task and because of the James infarction has been transmuted to the
Patrick, and he has also advice them to use the protection using the sexual activity. When the
Simon meet with the Patrick and the James with the cisent arranges a consultation at the
Terence Higgins trust who arranged the blood test for the both.
Advocate Roles and responsibilities
The role of the advocate is to offer the support to the person to those who feel they
are not being heard, and they also helping to ensure that they are takes seriously and that
their rights are respected (Price and Reichert., 2017). It is also to assist people to access and
understand appropriate information and services.
Their roles is to defend the person for whom they are taking the case, their roles is to
provide the justice to their client and take them toward the proper conclusion.
Advocate meets the brand's needs
brands needs and advocate in her life because she is suffering from the mental illness
and other problems. She need advocate so the advocate can listen to her and understand her
needs and her views and opinion. This will help the Brands to overcome from their mental
problem and the advice from the advocate will be useful for her ashe she can be normal in
their life and overcome from their mental problems which she is suffering.
GP Role and responsibilities
The role of the GP in safeguarding is wide ranging: recognition of patterns of neglect,
referring in a timely and appropriate manner to secondary health care colleagues or social care,
responding to inter-agency requests, supporting families and giving context at case conferences.
treat all common medical conditions and refer patients to hospitals and other medical
services for urgent and specialist treatment. They focus on the health of the whole person
combining physical, psychological and social aspects of care.

Role and responsibility
As per the all cases study GP has played their roles properly, and they provide the proper
advice and the treatment to the Patrick, brands and the maria and try to fulfill their needs and
their demand, their was an huge role of the GP in each of the case study and we have seen that
they huge provide proper treatment and consultancy to the each person individually and
properly which satisfy their needs and demand from the GP (Moghaddam and et.al., 2016.
Statement from the Advocate about the needs of Maria
Advocate can help the maria by understating her situation and her mother situation. Her
mother is having the decrease the used to awake at the late night which affect the Maria 's
persona life she is not able to attend the class. The advice from the advocate can help her to
get the professional healthcare for her mom, and she can also continue their work because
the profession healthcare team will help to treat her mom and help her to be get well soon.

Confidentiality in health and social care settings means that the healthcare professional or
practitioner should focus upon keeping a confidence between patient and themselves.
Confidentiality is one of the main parts of their care practise. It is extremely important for
practitioners to maintain a confidence between them and their patients so that they can focus
upon building a trust between them and their patients. This helps the patient to confined within
the healthcare practitioners that they would not share their personal details with anyone.
There are various factors because of which it becomes important to maintain
confidentiality between patients and practitioners. Some of the main reason because of which
confidentiality becomes important are: it helps in developing trust within patient that they can
share their sensitive information with professional (Hibbard, 2017). There are various kinds of
healthcare policies based upon confidentiality which restricts the professional for sharing
information with other’s if professional breech this policy then they have to face some sever
consequences about it. All the health care professional need to keep themselves updated about
data privacy laws such as data protection action and human rights act. Remaining updated with
current data privacy legislations helps them to maintain the confidentiality between them and
their patients. In order to adhere these laws, they should keep all the sensitive information about
their patients in a password protected document. they should only share that information with
people who are entitled for it.

All the information shared by the patients with their GP should be managed and stored in a
secure manner. it is extremely important to collect all kinds of necessary information of the
patient as it helps the GP to provide appropriate and accurate treatment to the patient. It also
helps in ensuring effectivity of the treatment (Handayani and et. al., 2017). There are various
kinds of information that can be collected by the GP from the patients such as: their past medical
records, medical history, life- long disease, any kind of allergies, current medications, personal
information etc. Most of this information are collected directly from the patients and some of the
medical records or medication information collected from different kinds of test.
Use of information ethically helps healthcare professionals to take appropriate actions for
providing effective treatment without any issues or conflicts. It also helps in reducing breeching
patients’ confidentiality that can further result in increasing health risk (Helia and et. al., 2018).
Ethical use of information helps in in reducing chances of conflicts also helps in improving
healthcare quality through system. In many critical cases it is extremely important to use
patient’s information ethically as it helps in preventing chances of fraud and reduces chances of
conflicts or issues. In order to ensure this confidentiality of information there are some multi-
disciplinary teams who works together in ensuring that information within health and social care
setting is use ethically.

AS per the report has been discuss the three case study which was about the Patrick
Green, Brenda and Maria. It has been explained the problems faced by this all individual. This
report has been concluded the types of institution who can help them for the treatment and also
has been concluded roles of multidisciplinary team, Sexual health advisers and advocate, GP
and also link with the case study.
