Introduction
 Integrated care plan is planned with that persons who works together to understand
the care provider and service user, keep them in coordinates and control and
provides the services to get the best outcomes.
 There are five elements in integrated care plan which is context, infrastructure,
financing, governance, leadership, engagement, and communication. In the case of
Mrs. E we are going to discuss about the integrated care plan for Mrs. E.
 In this report going to discuss about the framework or model of care plan which is
used during the care plan of Mrs. E. Achievable goal by the care plan is including
in this report.

What is integrated care plan
 Care plan is the presentation which carry the information that easily explain the
services as well as support which can be given to the patient. The integrated care plan
help to make the supporting planning for the patient with analysis of their
complication
 Mrs. E is a lady who lived with her husband Mr. E, who is suffering from the various
diseases like arthritis, hearing problem and dementia which means short term memory
loss.
 Mr. E uses walking sticks and hearing aid but due to the disease short term memory
loss he forget to put hearing aid and also forget to use his walking sticks.

 But after some time Mrs. E also diagnosed some serious
issue like vascular dementia, glaucoma and heart diseases
which needs daily medication for her.
 So that now Mrs E is unable to provide take care for her
husband and herself too. Therefore, Both of them required
some integrated care plan (Ferrara, and Morando, 2018)
such as health system, self-management support, delivery
system support and clinical information systems.

 In the context of Mr. E she is suffering from dementia in which she forget to take their medicines and other thing
so, that she need a care plan like:
 she need some help to stay oriented, calm and focused.
 Help Mrs. E with memory loss.
 Mange the sleep problem and behaviour.
 Talk to Mrs. E on regular basis.
 make her dressing and grooming as well this can help her to keep clean or tidy.
 Encourage her activities which are both like enjoyable and stimulating (Masson, Forrest, and Jopson, 2021).

 In case of Mrs. E if eye drop are not sufficient then take some
oral medication like carbonic anhydride as prescribed by the
experts.
 Mrs. E has is also suffering from some heart condition and
she needed to take medicine in night but she dis unable to
take that medication due to her worst condition therefore she
needs some integrated care plan by which these problem can
be solve

Justification of choice of model
 Biomedical model of care is used to treat diseases and it priorities biological factors excluding
environmental, social and psychological influences. It is a very common way used by doctors or health care
professionals to treat biological disorder. As Mrs. E and her family were suffering with dementia and other
biological disorders this care of model is applicable and can be used for diagnosis.
 Environmental model of care talks about environmental factors which affects health. Mrs. E was suffering
from dementia and heart disease thus it is required to eliminate environmental risk factors which are
injurious for her health condition.

 Social model is also known as person centered model of care it
priorities patient's needs and focuses supporting patient suffering with
dementia.
 It evaluates all the elements which affects patient's health such as
cultural., political, social and environmental.
 In given case study Mr. E was a dementia patient and social model
care focuses of patient's cognitive needs instead of physical or
medical needs.

What would be the assessment outcomes?
 By the assessment of report of Mrs E , these are the following
outcomes: As it is assess that Mrs. E is suffering from the heart
condition and glaucoma due to this she required to take medicine
at night time but she forget due to the problem of vascular
dementia.
 Therefore Mrs. E need some assistance from caregiver therefore,
she is able to differentiate between the non reality base and reality
base thinking. She will be able to adopt the explanations of
inaccurate interpretation in the environment.

 independence in the activities of self care
 providing intimacy and socialization, improving the communication
 decreasing the anxiety and agitation
 promoting the safety
 supporting and educating the family members of Mrs. E and to providing the

husband, who is suffering from different diseases like

supports for doing his work which is given by his wife Mrs.
E.
 But now she is also suffering from vascular dementia
condition, some heart condition and also from eye diseases
like glaucoma. Therefore now she is also unable to provide
care of her husband.
