INTRODUCTION
Day care services for health care patient is involving various facility this is the king of
holistic care facility. Under which service use get all type of services like social support,
refreshment and health care checks. Manager of the day centre have to take care of each and
every patient needs. If the user are showing any health care issues and symptom's then next step
is planing for the primary, medium and long term needs for care (Jennifer and et. al. 2019)
. Primary care is the initial step for the prevention of health issues by providing the health care
education and awareness about the health safety precautions. Secondary care is involving the
specialist focused care of the person due to their physical and psychological health requirement.
Long term care is involving the application of different variety of service's, which helps in the
management of medical and non medical needs of person. In this report care plan is going to be
develop according to the patient medical symptoms and requirement.

further discussed in brief by multidisciplinary team meeting in order to formulate management

further needs of patient diagnosis related to their strength of managing treatment. According to
the R. H. Jack (2016) it has been analysed that with the patient who is having the mental

primary care referral patient have to go through from NHSBSP referral screening in NSH Brest

basically involves post surgical side effects management practices in home based care.
Medium term care- This is the process of care which least for the some selective period
of time in-order to achieve some selective health improvement objectives. Such as in case of the
cancer surgery this process is implemented to provide care until their patient get overcome from

unconscious behaviour of the patient.
Team work- For effective holistic and long term care we as health care professionals
have to possess the skills of team work. Such as for the Gretchen Hoffmeyer holistic care along
with the management of variety of care facility's according to the specific requirement of the
patient I had to perform with the professionals health care team. With the help of my team work
skills I have achieved success in patient needs assessment.
Flexibility- In context to the mental day care service provider, As a care coordinator I
have to manage and provide best health care services along with the social, emotional care
practices due to which I have to be flexible through which I can manage each and every
responsibility. For Gretchen Hoffmeyer health care needs assessment and care plan

development, I used my work flexibility skills due to which I managed all need assessment
practices.
Empathy- As day care centre manager I have to be very kind and helpful because in the
long term care process we have to face various complication and risk if the harm. Empathy is the
quality if individual to provide effective care without the self concern and patient management
complications.
Analytical – One of the most important skill is the analytical skill which helps to
effectively analyse the patient requirement and further more health care needs I can do proper
need assessment and care plan development with the help of my analytical skills as I have used
in Gretchen Hoffmeyer care plan development (Shailaja and et. al. 2018).
CONCLUSION
From the above study and analysis it has been concluded that health care organisation and
management is involving various sets of the rules and guidelines according to which specific
problems of the patient is manage, control, cure. In the case of day health care service provider
of mental health disorder. Nurses and managers have to take care of all medical, physical and
emotional needs of the users. Such as they have to facilitate effective, short, medium and long
term care of the patient. In the situation of any other health complication that is the responsibility
of day health care manager to provide best care with care planing. There are various health care
model and systems according to which effective patient needs should be asses along with the
development of proper goals of the improvement and control. In context to the breast cancer
and bipolar disorder there is the various needs of the users which has to we facilitated by the
manager such as the NHSBSP diagnosis and screening for proper understanding of the patient
breast cancer type, so that effective care plan is can be develop. The care plan for patient is
involving improvement goals and care practices along with the treatments, such as the
Radiotherapy treatment, healthy environment, social, personal and emotional support. There
should be proper short, medium and long term care with specific goals. This care plan have to be
adjusted with proper evaluation of the intervention impact and result. In order to provide the
effective care there is need of some professional skills which should be posses by the health care
managers and nurses such as the communication skills, Team work , Flexibility, Empathy,
Analytical.
