Description
This task provides you with an opportunity to develop and demonstrate knowledge of
processes associated with assessing a person’s mental state in different health care contexts,
and supporting the person to navigate to a place of recovery.
Assessment Criteria
The workbook responses will be assessed according to the following criteria:
 Use of evidence to interpret and analyse the mental state assessment, including an
assessment of risk
 Interpret and use evidence to plan care to support the mental health needs of the
person in diverse health settings
 Critical appraisal of principles of equity, rights and access
 Information literacy skills: use of sources and citations
 Written presentation including structure, grammar, application of referencing protocol

with people with mental illness. You must adhere to the word limit provided for each

1. Define the mental state assessment, describe when it is to be used and describe why it is a
useful tool for nursing or midwifery practice (max 100 words) 5 marks
Answer:
Attitudes, patterns of behavior, temperament, emotion, expression, thought process,
thought output, vision, awareness, and perspective in the fields of appearance and
performance is carried out as mental state assessment which is apparently a systematic
method of measuring and explaining the current thinking of an individual (Riecher-Rössler,
and Studerus, 2017, p.209).

The aim of the Mental State Assessment is to obtain a full cross-sectional
representation of the mental wellbeing of the patient and this helps the clinician to make an
accurate diagnosis and action plan formulation. When formulating these treatment plans, the
patient's biographical and previous medical history also helps and thus needs to be kept in
consideration (Eisenacher et al.,2015, p.3329).
2. Describe Andy’s appearance and behaviour during the initial assessment (100 words) 3
marks
Answer:
Andy's presentation was a bit clumsy, with beard trimmed and hair cut appropriately.
He came across wearing a casual outfit and, while responding to certain questions, had a
shaky attitude. Andy's reaction to Dr. Taylor's questions was right and straightforward, but
his answers to the questions regarding his addictions were somewhat inconsistent as if he
were not prepared to share the details. Certain indications are common regarding the
occurance of schizophrenia (Bansal et al., 2018, p.168).
In addition, the presentation clearly showed that he also had a trust problem with his
counselor, Dr. Taylor. Even at the end of the tests, he asked her either to have faith in him or
not to have real feelings and thoughts (Kleiger, & Khadivi, 2015).
3. Differentiate between mood and affect, and then describe affect as it relates to Andy (max
50 words) 3 marks
Answer:
The impact is typically more used as a noun, with two variants of very similar
significances. Affect can mean shifting an individual's state of mind or altering an
individual's emotional perceptions. Mood is typically a rapid, emotional process arising from
either a specific stimulus or environmental influence (Tang, & Pinsky., 2015, p.54).

In Andy's case the result showed that he sometimes feels more aware that he needs to
carry protective gears for his defense such as knife or baseball bat.
4. Discuss Andy’s speech (max 50 words) 3 marks
Answer:
Andy's voice differed as Dr Taylor's proceeded with her questions. He responded
unhesitatingly at the beginning of the evaluation however, he often losed his eye contact,
when Andy was asked about the history and background of his problems. At moments, when
he was questioned about the medications he was taking, Andy refused to answer the
questions. That was because of too much cannabis use which he did not want to disclose
(Corcoran et al.,2018, p.69).
5. What signs or symptoms associated with an episode of psychosis can you identify in the
interview with Andy? (max 50 words) 5 marks
Answer:
The signs that came to light when Andy was interviewed were (DeVylder ety al.,2015, p.489-
493):
• He had been too confused about his surroundings.
• He also had signs of listening-sound hallucinations.
• He had propensity to bring tools, such as knife and baseball bats, for his security.
The signs suggest that(DeVylder ety al.,2015, p.489-493):
 He was unknowingly schizophrenic and can become aggressive on anyone around
him whom he feels threatened by.
 He could even threaten to have suicidal tendencies.
6. What tests might Dr Taylor request at the end of the consult with Andy, and why are these
important in the context of symptoms consistent with psychosis? (max 100 words) 3 marks
Answer:

Dr. Taylor's findings were: Context evaluation of the situation, Risk assessment, Drug
background assessment and insight assessment.
Based on the risk assessment results, Dr. Taylor may ask for the hormonal alteration
test, which may suggest his chances of becoming aggressive and having panic attacks due to
anxiety. However, based on the evaluation of the history of the drug, other examination that
can be taken into account is the family atmosphere check and Thematic Apperception Test
due to the heavy use of cannabis without any obligations (Stein et al., 2015).
7. Dr Taylor assesses Andy’s thought. Describe how you think she might describe and
document her findings in the progress notes? (max 50 words) 5 marks
Answer:
Dr Taylor will list certain conclusions on the basis of her evaluations The four tests
put into action enabled her realize this:
• Andy had distrust of his surroundings.
• Typically he holds the baseball and knife bat as defense.
• He often has thoughts in memory, thus the signs of halucinations are counted.
• His confession of having confidence issues concerning his parents indicated that the
condition can worsen even more.
8. How would you assess Andy’s insight into his current emotional and cognitive state?
What are the likely positive or negative outcomes associated with this level of insight? Please
comment on his overall judgement as well. (max 50 words) 3 marks
Answer:
The cumulative conclusion says:
• The derogatory word continues in his heavy abuse of drugs.
• That Andy has a positive thinking because he is mindful of his circumstance and he
reads a book that talks of conditions similar to himself.

9. Looking at Andy’s presentation overall, what risks does he currently present with? Are
there risks that you have excluded? Provide evidence for all your responses. Please give
consideration to his physical health as well as all other possible risks. (max 150 words) 5

Answer:
The overall situation indicates that Andy is too nervous about the things around him.
Apparently the conversation notes states that he has trouble trusting almost all the people
around him. He also admitted that he always felt he needed to defend himself also from his
parents.
Thus the dangers prevalent in the cetrtain situation are (Manseau, and Goff, 2015, p.819):
• Serious levels of visions
• Severe chances of having anxiety thought process because of the fears that some force
is watching over him.
• The risk of psychological problems leading to have tendencies to suicide in such
situations is very high.
• Risks of aggression against others are also prevalent due to lack of faith.
10. Andy is diagnosed with a psychotic disorder. What interventions/treatment and care
pathways/options would be appropriate for his care? Give consideration to the setting in
which this care might be provided. Provide rationales for your plans. You will need to read
the relevant chapter in your textbook, as well as look for other academic sources (max 300

Answer:
For people with a psychiatric disorder the most popular treatment is therapy with a
class of chemicals called antipsychotics.

Antipsychotics help in minimizing depressive symptoms of psychiatric disorders like
schizophrenia but are not totally controlled or healed by themselves by underlying psychotic
illnesses.
The so-called second-generation antipsychotic are used by medical professionals
more commonly in order to treat psychosis Although they are commonly used in the United
States, this is controversial It is not approved by the World Health Organization (WHO),
except only for clozapine (the US Clozaril brand and FazaClo), which can only be used under
strict monitoring if other antipsychotic drugs have not been messed with.
Andy's actual condition implies that his existence and events with him are quite
unpredictable. The care plan also suggests that its services should be carried out in a holistic
way, engaging families, friends and colleagues in order to gain trust. Therefore, he needs the
help of medical professionals to make him understand that his depression with respect to life
was immoral, which can be done through the use of Cognitive Behavior Therapy (CBT).
Person Centered treatment is a health care strategy that focuses mainly on the person
they support. This attention must take into account a very unique experience, context, goals,
and priorities alongside Andy's physical and mental health needs to be preferred. He will be
guided in achieving his full capabilities–physical, biological, economic, intellectual,
cognitive, political, practical, mental, company, recreational, personal, moral, and social–in
his life. Dr. Taylor will provide concentrated attention and support him (Hungerford et al.,
2018, p.54).
The healthcare professional should collaborate with a person-centered approach to
allow patients to make up their own minds about healthcare. Thus Dr. Taylor engages
actively in the evaluation process (Hungerford et al., 2018).
