INTRODUCTION
Health care is related to treatment, care, after care with illness, injury, disease or
disability. Social care is associated with day to day work such as personal care, social service,
protection and provide accommodation like care home. This study will be based on health and
social care. This report will describe the local community resources and provision that supports
integrated care planning. It will discuss the local unmet need related to healthcare provision in
own locale. It will evaluate local resources to meet the needs of an identified individual requiring
multidisciplinary care.
Outline local community resources and provision that supports integrated care planning/working.
Integrated care is an organist ion which is based on principle of delivering care to achieve
improved patient care. Integrated care workers are supported by the social care and local
government that focus on keeping well patients and people with that they provide tailored,
responsive care at the time of illness (Chassin, Mayer and Nether, 2015). According to
definition of world health organization integrated care origination are responsible to delivery,
organization and management related to treatment, diagnosis, care, health and rehabilitation.
Local community resources supporting the integrated care and supporting working group that
includes local government association, NSH commission board, monitor and department of
health, association of directors of public health.
The increasing prevalence of people and the aging of people with long time conditions
has changed the way of care and health are provided. Financial resources are limited for local

adjustments to affect positively the service variability. Due to these people will attract more
toward healthcare system. Considering this, local unmet needs related to health care provision

From the above study it has been concluded that local community resources and
provision supported health care in order to provide better services to people. It helped health and
social care worker to deliver services groups and individuals. Awareness of health care in public
assisted them to engage more people in social service.
