INTRODUCTION
Multidisciplinary care can be referred to as an effective team approach which pertains to
HSC industry. It usually determine treatment of opinions and planning by making use of a
collaborative approach inclusive of HSC service concentration upon patient and their families
(Cheng and Tsaur, 2012). This report is based on case study of Jonathan Tertullien who is a 80
old man diagnosed with early stage of Vascular Dementia. Additionally, this report is going to

needs and differences in social and healthcare renderers.
TASK 1
P1 Identifying locale resources and provisions assisting ICP
Within today's context, locale community is often an array of opportunities for general

making an individual healthy(Bartenieff and Lewis, 2013). According to the given scenario
Jonathan Tertullien, was affected by many diseases such as early stage Vascular dementia which
makes him forgetful, unhealthy heart condition and high blood pressure. Due to such problems
he was not able to take of himself. For this, adult service providers are adopting different
measures through which integrated care planning can be executed.
There are a number of organisations which provide help out to match interests and needs
of people who are suffering from disabilities. Here, the first step is to identify the interests of
people who are willing to get support and aid. Community is making use of manpower, that is,
human resource in order to determine the local areas whereby people are affected by diseases.

of medical needs and wants of people (ChenLi and Chen, 2013). As per the case scenario, an
eighty year old man was suffering from various diseases and he was not able to take care of
himself. Moreover, adult service provider need to critically understand gaps in healthcare and
social support in order to help out such patients who are alone are not able to take care of
themselves. Eventually, for such purpose the need to examine what are the needs for such heath
care services as well as social care services in which they are lacking behind. The community is
usually focusing on an individual characteristics which is associated with unmeet demands of
local people. They are experiencing such unmeet demand with relation to mobility task,
examining daily living needs of such person who are suffering from uncured diseases as well as
instrumental activities of daily livings. Thus, a community should need to examine unmeet
demands for needy of healthcare and social services patient in assessed and systematic manner so
that an adequate comprehensive care can be provided during course of diseases an individual is
suffering.

In today's era, individuals utilise HSC facilities with the passage of time. This needs of a
person may differ from living stages and types of situations that have occur at any part of life.
The HSC facilitates are renderd by several care providers such as NHS, NSPCC and Priory Adult
Care.
NHS (National Health Services), these are those services which are provided by
government. They are providing government aided adult medical amenities inclusive of nursing
as well as care homes and day cares.
NSPCC is providing services to general people by not for profit organisation which are
usually found to be register charities (Bott and Spillius, 2014 ).Its main motive is to provide
healthcare and social care services to needy ones.
Priory Adult Care, they are making real difference from other HSC amenity renderers.
They are providing range of amenities specialist for residential as well as support living services
for adults aged over 18 years over a learning disability care in later life.

D1 Evaluation of domestic resources for of individual needs
Adult service provider are using different local resources in meeting up individuals need

scenario, An old man was suffering from many deceases for such purpose he was not able to take

From the above report it has been concluded that multidisciplinary is an approach that
help general people to involved in recreational activities in order to develop relationship with
others. Moreover, they are various organisation and association which usually help out to match
interest and adopt resources to have people who are suffering from disabilities. Thus, adult
service provider are determining difference between receipt of actualised amenities that depict
measurement of success of HSC.
