INTRODUCTION
'My Health Record' is the secure online synopsis of an individual health information and
which is the project of The Commonwealth Government. The project funding has been approved
and in this report stakeholders of this project will identify and positioned with the help of
stakeholder mapping. The role names of stakeholders who will be provided access to the project
will be identified and explanation of their interest will be provided. Questionnaire will be
provided to one of the stakeholder in order to obtain information regarding issues they faced
regarding patient medical information. Eventually use case model will be prepared for the
information system.

Stakeholder analysis helps in identifying the key stakeholders and their relationship with
the project (Knight and et.al., 2014). In the project 'My Health Record', numerous stakeholders
are attached who are directly and indirectly influence the success of the project. In this context,

role. They have to assess the information and record it on the online database. CEO and Director: They have interest as the project will aid in the growth and
development of the hospital by reducing extra cost and efforts. General managers and managers: These stakeholders have interest as it will assist them
in providing health care professionals precise information about patient health
information (Rabbi, Mamun and Islam, 2015). Technical Staff: They are essential in order to rectify any error identified in software and
applications. Patients: They are important and have high interest as it will help them to retain all the
information regarding their health check ups, medicines, treatment, etc. Employees: Employees who make contact with patients will be able to get access the past
information about the patient and provide them information regarding which health care
professional they need to consult with. Health Care professionals: They have high interest in the project. This assist them in
acquiring all the information related with past medication and treatment of patient and
can provide them precise treatment thoroughly (Scott, Richards and Caldwell, 2015). Organisational service providers: These are medical suppliers, raw material providers,
etc. They have low interest as they will provide medical supplies according to the need
and requirement. Society and community: Society and communities have high interest as they will be
greatly benefited by the project.
Health insurance: Health insurance providers will be able to retain the information about
client past medical record which helps them in managing their claim process.

To get more in-depth information regarding the environment, the area of problem and the
way system would operate to eliminate the problem a questionnaire comprises 10 question will
be provided to the health care professionals of the health care institutions. The motive of the
questionnaire is to identify the issue faced by these professional before providing treatment to the
patients regarding their past medical record (Robertson, 2014). In this context, the questionnaire

medication information of the patient (Elliott, Lee and Hussainy, 2016). Without patient consent,
doctors or health care professionals are not allowed to get access to the information. After giving
the consent, the doctor can access the information, provide medication and treatment accordingly
and update the information after the treatment. Eventually, doctor and both patient can share the
updated information to other hospital where he or she will get the treatment (Scandurra,
Pettersson and Hägglund, 2017).
