Definition of team-based healthcare
To define team-based healthcare it can be said that it is the opportunity to provide health
services to the individuals by the collaborative effort of more than one or two health
professionals to ensure that the patient is receiving the healthcare of the quality that is on the
higher end (Khoury, Iademarco & Riley, 2016).
Team approach in NeighborCare Health
The Neighborcare Health is enriched with having a team approach in the workforce
where the doctors, nurses and other health professionals work as a team and they have a shared
belief and ambition to give the best healthcare to the individuals. There are some advantages of
having team approach in the health organizations like NeighborCare and those are mentioned in
this section.
Having a team approach in the organization, the patients and the doctors benefit the most
and undoubtedly in a health organization these two are the most important components. In this
specified organization initially some of the doctors were skeptical regarding this issue, but later
on they realized the importance of having a team approach (Jacobs et al., 2015). It is a matter of
fact that not all the employees of the organization are equally competent in their working field
but after they started working as a team, the number of errors have been decreased and the
number of positive outcomes regarding the ailment of the patients has been significantly
increased and that earned the organization immense reputation in the market of USA. For an
example, in NeighborCare when a diabetic patient comes a foot examination becomes necessary
at times, and without a doctor’s intervention, a nurse can easily take out the operation

chart of the patient without help from the doctor. The doctor then quite easily does his or her part
of work efficiently. In this way the accuracy of work also gets increased.
Three insights
The team approach will help the doctors to take more time in determining the reason
behind the issues of the patients and he or she would be able to invest more time to work on the
issue of the patients.
Having a team approach in the workforce of a health care organization all the
components of team; like nurses, doctors, dieticians and other health professionals will get ample
time to accurately take out their job and rate of errors will automatically decrease (Lobelo,
Stoutenberg & Hutber, 2014).
The individuals who will visit the healthcare organizations seeking assistance regarding
their health issues will immensely benefit from this approach as this approach will allow them to
get all the assistance in each step of his or her treatment from the specialists in those fields.
Review of community assessment and feedback:
The community needs assessment reveals that the Caucasian people and American people
are under the highest risk of diabetes. The review clearly shows that these two groups require
urgent help and guidance from medical practitioners like doctors and dieticians. One can
recommend that these people manage their lifestyle patterns and food habits ("Washington Life

Day 3:
The community need assessment can be summarized as, that people of different age and
races need to be concerned about the rising threats from diabetes. The three insights that can be
pointed out after discussion are the people should be concerned about their health, the
government, hospitals and NGOs should counsel the people suffering from diabetes and the aim
should be to raise awareness among people who are yet to get the disease to manage their
lifestyle to avoid the disease (Lobelo, Stoutenberg & Hutber, 2014).
Day 5:
The two constrictive recommendations are:
1. The female population of the US should manage their lifestyle to keep control over diabetes.
This is justified because the gender pie chart shows that females are more prone to diabetes.
2. The government should cheap diabetes care to the people and raise awareness among them to
fight lifestyle diseases like diabetes. This is justified because it help them to be aware and
modify their lifestyle to minimize the diseases.
Conclusion
Thus to conclude, it can be said that having a team approach in the organization like
NeighborCare Health will gradually improve the quality of the medical assistance they are
providing to the patients and in this way the professionals of the organization will be able to
perform their tasks with more accuracy.
