Introduction
The nursing medical note is the medical note or the medical healthcare record which is
kept by the nurse which provides an effective reflection of the assessments of the nursing,
the changes in the conditions of the patients, the provided care and all other relevant
information for supporting the clinical team.
The thesis statement is that ineffective nursing progress note can hamper in providing
effective care to the patient.
The given case study is about a 73 years old aboriginal male who was about to get a
transition care but because of the lack of communication he has not received such care.
What are the importance of literacy and communication in healthcare?
The health literacy and effective communication in between the nurses and in between
the patients and the doctors is one of the key aspects in the improvement of the health and
also in the improvement of the quality of the care which is provided to the patients.
Literacy refers to the ability of any person to read or to write and the communication is
the way the nurses speak with the patients.
Effective communication provides the care providers and the care takers to receive the
highest quality of healthcare.
Using of non-verbal communication
In the given case study, the body language of the patient has described that he was really
very upset about the fact of being transferred from his own country.

His slumped body posture showed that he told that he told the truth that he had not eaten
anything since the last three days
The nurse was able to note down in the progress note about the body language and
activities of the patient but she has nor mentioned anything about her own non-verbal
actions, the care she showed to the patient.
The nurse could have shown such care to the patients so that he would have felt secured
in the new place. He had already reported that he has been feeling very upset in the new
age care facility and also he was not getting proper care in the new place.
Using of verbal communication
In the given case study, the nurses had been able to note in details about each and every
details of the statements of the patient but nothing has been mentioned about the way the
nurse communicated with the patient.
The nurse should have communicated with the patient politely and should have used such
languages which can increase the confidence level of the patient.
The transition care required by the patient to progress
According to the information from the progress nursing report, it can be said that the
patient requires both the physical as well as mental care. The progress report was not
written in details so it hampered in taking care of the patient.

Conclusion
The thesis statement is thus proved that if the nursing progress note do not contain detail
information about the health and the mental issues then it is not possible to provide the
patient with effective care and especially when the patient changes the aged care facility.
The communication as well as the patient education is also important in order to make the
patient feel the need to understand in details about the issues from which he is suffering.
