Registered Nurse.
You go with Rob to the treatment room where Mr James Patterson (78 years of age) has an
infected wound on his left lower leg following an injury he sustained on his farm. Mr Patterson has
always been a farmer and is very independent. He is still active in the farm and likes to tinker
around in his shed when he is not busy in the farm. He lives with his wife on the farm which is two
hours from the clinic. Their only son emigrated to Canada 5 years ago and they remain in touch
via video calls. They especially look forward to chatting with their grandkids who visited them 2
years ago before the pandemic hit. Mr Patterson has a history of type 2 diabetes. Mr Patterson
has a sweet tooth and is fond of his wife’s baking. He always gets a cake for his afternoon tea and
he loves his lollies. He thinks the little wound won’t keep him away from his work and tools.

The social determinant of health is defined as a condition which is well associated with the
economic and social condition which usually influence the individual health and also create
the group differences in the health status. Moreover, in this report, the concept of the social
determinant is well analyzed because there are some factors which influence the health
status of James within the case study. In the case study, the social determinant of health
which is analyzed are usually education access and quality health care access and quality
neighborhood and built environment and also social and community context that render the
optimum health outcome of the James (Social Determinants of Health and Related
Inequalities: Confusion and Implications, 2019). In this, the education access is usually

showing the elaboration taking time to diabetes in the casual way where they continue to do
their farm activities without healing the wound that they face during the farming. In this,
they require time to heal the injury because of type 2 diabetes where the healing factors are
become slow due to the sweet tooth. The other social determinant is health care access and
quality because from the farm to the clinic the distance is approx. 2 hours which is highly
measurable in terms of emergency. James do not have any neighborhoods and environment
that may not create any social group and community where he usually associated with their
child with the video call who is immigrated in the Canada (Al-Mrabeh and et. al., 2020).

a. It is very important in order to taking the perception of James and his wife in order
to plan and provide the support and care which is related with the healthcare
because as a farmer, James facing to the issue in order to know the terminology
which is related with the health whether there also know about the complication
and consequence of diabetes and associated injury. In addition, James’s wife also
plays vital role in order to provide the support and care in terms of providing the
knowledge and education which is provided by rob, where they take proper
monitoring and inspection to his husband regards with the proper functions that

terms of diabetes and injury are usually taking into the consideration which may
helpful to provide the support in order to enable the healing factor as soon as

and his wife because due to the diabetes especially type two diabetes can reduce the
healing factor among the individual that is well analyzed within the James (Cannon
and et. al., 2020). The injury takes longer time to heal because of the slow
progressive factor which take in a prolong time to provide the relaxation and
healing approach towards the injury in such cases, James must reduce the farming
and taking rest in order to provide the accessibility of a long time of break from the
farm is helpful in order to reduce the complication of infection and other factor to
the injury.

and his wife that they take proper perspective into the account because of the injury
where the diabetic patient is usually taking a longer time in order to improve the
health condition due to the low hormonal factor or resistive mechanism that
contributed to heal the injury or the site due to the low healing factor. In addition,
the knowledge is well contributed for the both to make an approach and care plan

may play a helpful role for the James in order to preserve the condition of injury

farm. The infection is also higher in the injury site which must be prevented by

also experience that the nurse’s role and responsibility is to manage the condition and

study which I experience is lack of support from the social context, community and social

condition of health to the number of people who is facing the issue of illness or disease
