based on the care andservices receiving in the hospital. Some of the tools like information
packages and pocket cards have been provided to the staff members for smooth process of
ISBAR.

fatal diseases. Furthermore, the chosen nursing staff met the inclusion criteria invited to
participate in this study. Their consent for observational audit was received and 90% of them
return to the satisfactory survey in the before group and 57% what there in after group.
