Reliable, Proactive and confident in working with aged care clients and people with chronic
disorders:
I have good verbal and non-verbal communication skills by which I am able to be proactive and
confident in interacting with old aged patients and patients with chronic illness and thereby
develop therapeutic relationship with them. In the last healthcare department, I saw that most of
the patients were unhappy and spent most of their days alone in their rooms and felt depressed,
unhappy and affected by their situation. I identified from the situation that the patients were
mainly sad and depressed as most of them had lost their independence and had to depend on the
professionals for their activities of daily life. The nurses only performed their own duties and did
not involve in effective communication with the patient apart from their clinical duties.
Therefore, I took the initiative to advocate on the behalf of the old aged patients and requested
the nurse leaders about an importance of training required for the nurses of the ward to develop
effective communication skills and the need of developing therapeutic relationship with the
patients. The nurse leader was impressed and she held training classes of the nurses of the ward
training them show communication and therapeutic relationship shoes empathy, compassion,
respect and trust in the relationship with patients. This resulted in the nurses develop proper
skills that helps in changing the work environment within the healthcare. The patients were seen
to be happier in their wards and this had positive outcomes on their health. They were highly
satisfied with the treatment and care that they received from the nurses and work of mouth
helped in developing the reputation of the healthcare centre.

management:
I have the ability to adapt to various situations within a team, identify the issues that a team is
facing and develop interventions that develop the work output and service of the team in the
healthcare centre. While working in the last organisation, I was appointed as a care coordinator
for a multidisciplinary team that was supposed to attend stroke patients who require
rehabilitation. However while working in the ward, I realised that there was power struggle
among the junior and senior nurses, doctors, experts, therapists, dietician and many others. As
result of this, the care given to patient was not proper as trust, bond effective relationship and
communication was absent among them. I took an initiative and reported this to the higher
authority who arranged for several seminars of stalwarts to make them understand the negative
outcomes of such power struggles of the patient. Different types of medication error, diagnosis
error, recommendation error, miscommunication and many other takes place that may affect the
health of individuals. I also arranged for feedback giving and receiving sessions in form of
meetings where each of them discussed about their issues and provided constructive feedback to
each other. This helped the team to overcome the power struggle, establish relationship with each
other and thereby work collaboratively to provide best care to patients.
Ability to follow as well as implement a designated plan to achieve a desired outcome:
I have knowledge about various types of theories in nursing like that of Levett-Jones clinical
reasoning cycle, Watsons nursing theory of caring, Roper-Logan-Tierney Model of Living and
many others that help me correctly to judge a situation, taken proper cues, realise the priorities
and thereby develop effective care plan for the patients. In one of the organisation where I

worked, I often had to treat patients who came with chronic and co-morbid situations. It often
used to become difficult for me to understand from where I would start my diagnosis, how to
realise the priorities, how to care for the patient and others. In such situation, I used to keep
myself calm and did not panic. I used to apply the clinical reasoning cycle where I used to follow
each of the steps respectively .for proper documentation, I used to follow the ISBAR format. My
ability to apply the correct theories at the correct times and analyse the results of the theories
helped me to identify the care priorities. Accordingly, I used to design my care plans for the
patients, therefore the plans always used to be successful and ensured that the patients not only
overcome the illness but also have lifestyle management education programs that provided them
with better quality lives.
