Nurses have to maintain different values and the beliefs during the treatment of
patients. Attitudes, assumptions and mindset of patient can influence nurses’
behaviours. Decision making in relation and impact of judgement are important for
evaluating patients’ conditions. After observing different case studies the professional
career of a nurse will develop in a better way. Transformational learning is one of the
process of constructive and deep meaningful learning that will help a person not only
theoretically but also by acquisition and support them in practical field. The
assignment will show introspection and the proper analysis of case studies by
improving their own beliefs and promotion of transformational learning (Melrose,
Park, and Perry, 2013).
Alex Abraham is a 42-year-old man who was
diagnosed with type 2 diabetes recently. He
works out of the town for 12 hours shift and has
to stay away from home for two weeks. He is
married and has two children. He has difficulties
to accept the diagnosis.
Mr Alex is chosen as I have experiences in treating
diabetic patients. The present condition of Mr Alex
can be managed with the support of patient’s family
and my knowledge. The self-management of
diabetes can avoid long-term health difficulties but is
often complex and hard to accomplish. Patient’s
autonomy for managing their diabetes is more
effective for self-care and glycaemic control.

• Ask him for meal plan and proper
ratios of nutrients will be informed.
• Physical exercises will be suggested
for better physical condition and
outcomes.
• I will take help of genogram and
ecomap to find the risk factors and
support factors.

educate him about self-management of
diabetes.
• The medication regimen and lifestyle
modification routine should be

Hailey Berry is a 66-year-old woman,
diagnosed with cervical cancer. She and
her partner of 30 years met oncol ogist.
They want review of pre-operative
checklist with nurse.
I have experiences in oncology unit. I can give support and meet their expectation, as I am able
to understand their physical and psychological condition. I have also experience to treat
homosexual patient. I do not possess any bias towards same sex couples
The patient and her partner are stressed about the diagnosis and
surgery. They might be anxious that they cannot get better care
due to their LGBTQ status and ethical differences.

Patient 3: Fiona Friday
Patient’s details
The reason I choose the patient
Assumption about the patient
Fiona Friday is a 16 year old girl and in second trimester of
pregnancy. She had never been in prenatal care before. She
does not live with her family. She live with a friend and her
family. Being convinced by her friend, she has visited a
weekly clinic in the community for prenatal care.
I chose Fiona as I have worked in maternity unit for some
years and have experiences of serving people of vulnerable
populations. She is undergoing with teenage pregnancy and
will have many behavioural changes as well. I want to help
these vulnerable people with clinical, social and psychological
support . Being a woman, I can understand her feelings and
can show compassion to her.

Fiona is a young girl and does not know much about responsibility
of being pregnant. She may be afraid to admit her condition and
trying to avoid the reality. Her parents may not support her in this
condition. She does not have adequate knowledge about pregnancy
and motherhood.

• I would focus on her family structure and her health condition as these two aspects
are related with her social context, communication, roles and beliefs with the help
of family reasoning web.
• I will use genogram and ecomap for assessing her strengths, support systems and
history.
• I will assess how Fiona is coping up with her condition. I will ask about her
lifestyle and evaluate whether she is prepared for living with a baby.
• I will discuss with her whether she is planning to put her baby for adoption.
• I will give her written information and teach her about pregnancy.
• The nursing professionals should inform Fiona and her friend about strengths of
every resources. They should be educated with the collaboration of social support
and groups and social workers.

Patient 1: Ernie Erbach
Patient’s details

Assumption about the patient
Ernie Erbach is a 29-year-old man
who undergoes from numerous
problems related to polysubstance
abuse. He has a summit with his
social worker today and has
entreated to see a nurse at the drop-
in clinic for general health-related
apprehensions.
I have no experience in dealing with
polysubstance abuse in nursing career.
I know that Ernie requests a lot of
supports and I know that I would not
be his best fit to get him the help he
needs.
A fundamental problem is affecting Ernie
to poly-substance abuse. There is most
likely a past trauma or primary mental
health concern that has not been
lectured or not treated correctly that is
turning Ernie to his substance abuse.

Patient 2: Joy Carr
Patient’s details
The reason I choose the patientJoy Carr is a 50-year-old man successful
through cardiac therapy post myocardial
infarction. He typically derives alone to the
sessions, other than being accompanied
by his wife on instance. He quitted
smoking and felt that his diet needed to
improve and his wife prepared meals
initially.
I would not choose Mr Joy as he is a self-
motivated individual to advance his
health by adjusting the risk aspects such
as he abandon smoking. His wife is also
taking part in the therapy program as
possible. I think they are on correct track
and are operating the decisions
accessible to lead a healthy life.

• His wife is supportive and she attends some health sessions and
help to modify risk factors ( stress at work, lack of exercise,
smoking and unhealthy food patterns ) for his health.
• I will gather all information from his wife and construct genogram
and ecomap to identify the risk factors.
• Identification of coping mechanism and learning how to balance
lifestyle are significant in healthcare treatment.
• Appreciate him for the active involvement in cardiac
rehabilitation. Cardiac rehabilitation (CR) is an effective approach
to improve the prognosis (Furze, Doherty, & Grant-Pearce, 2016 ).

Patient 3: Charlie Heron
Patient’s details

Charlie is a 5-year-old boy, is
admitted to hospital for acute
respiratory distress. Grandparents
of the patient accompanied him to
emergency as his parents are in
vacation.
I do not have much experience
in acute paediatrics and would
panic during assessment of
children who are in acute
condition. I feel that my
nervousness can make the
situation worse.
Patient’s family especially his grandparents are terrified and he was afraid, as
he is very young. However, there is no mention of inhaler and respiratory
history in patient’s family. Due to lack of information, I cannot diagnose
asthma or anaphylactic reactions.
Charlie’s breathing rate and oxygen saturation should be
checked first. Full body evaluation sould be done for better
evaluation. After that, vital signs and symptoms are observed
in patient’s body. Professionals inspect allergies and rashes; I
would also send the grandparents’ home with handouts on
how to use it in education and awareness. I would also
interview the grandparents to see what Charlie was doing prior
to the respiratory distress (exercise, new foods, exposure to
pets, dusts etc.).
