This report presents a research project focused on developing effective nursing care plans for dementia patients. Nursing care plan topic chosen for this research project is “to develop a nursing care
plan for dementia patient”. Dementia is a health disease that directly impacts ability of
a person to carry out daily activities. It further impact memory of a patient as well. Due
to this it is important to develop an effective nursing plan for dementia patients.

I want to know ways in which proper and effective care through effective nursing plan
can be provided to patients. I want to know effectivity of treatment which is being
provided to such patients, what role nursing care plays in it and reason because of
which mortality rate of dementia patient is increasing. But most importantly I want to
know impact of Covid-19 on care of dementia patients.

Results of this study clearly explains that nursing care plays a vital role in
enhancing overall efficiency and health of patient whether they are suffering from
dementia or not. This study clearly explains that residents who do not have dementia
require additional care as compared to dementia people.

These results can be used by Australian healthcare context as it clearly explains
importance of nursing care for individuals who are suffering from dementia. It also helped
in understanding need of nursing care for dementia patients in different sectors that are
required to be focused upon.

One of the main limitation of this study was that it did not explain percentage till which
nursing care is effective for dementia patients.

Putting it all together. What were the main findings of your literature review? How would
you use your conceptual framework to integrate your findings into the workplace?
According to the main findings of literature review it has been identified that there
are various factors that are required to be focused upon in order to develop an effective
nursing care plan for achievement of desired outcomes. It is important to identify or detect
dement or its symptoms at early stages so it helps in providing more effective nursing care
to patients. It is further important to analyse and identify needs and requirement of
dementia patients as it directly helps in enhancing overall health of dementia patients and
further makes it much easier to achieve desired output in an appropriate manner. It has
also been analysed from above literature review that in order to develop an effective
person- centred nursing care plan it is important to focus upon shared care and nursing
team. This is because shared care or nursing team can effectively help in fulfilling all the
main needs and requirement of dementia patients. These factors can help Australian
healthcare system to enhance dementias care and heath outcome required to be
achieved can be achieved in an appropriate and effective manner. It has also been
identified that in order to develop an effective nursing care plan it is important for leaders
to adopt an appropriate leadership skill. For this conceptual framework developed above
can be used. This conceptual framework clearly identified transformational leadership
factors that together can be used for developing a person- centred care framework for
providing effective care to patients.
