INTRODUCTION
Juvenile idiopathic arthritis (JIA) refers to a common type of arthritis in kids and teenagers
which can cause joint pain along with inflammation in ankles, elbows and knees and others. It
has also impact on individual body which can create negative health impact (Hechler et. al.,
(2021)). There is need to delivery proper care and treatment with combination of partnership
treatment which can be effective to the patient with JIA and asthma. Patient with JIA and asthma
can face various difficulty which needs proper care and in this there are some barriers which may
impact the patients care goal which needs to be tackle effective to provide effective care to the
patient (Polizzi et. al., (2020)). In this, there is need to deliver the quality care which can be
provided through the service of effective nursing staff. In this, there is need to ensure about the
relevant care coordination skills which are generally possess by the registered nursing staff are
able to provide those care service and manage it more effectively which can help to deliver
quality services to patient. In this, there is need to use some strategy which can be effective to
deliver the quality care services to patient for their health improvement (Schenck et. al., (2018)).
TASK 1
Patient Care Team
Patient with JIA and asthma need for the proper facilities which can be effective and allow
to deliver quality care to the patient for the health improvement. In this, there is need to ensure
about the facilities like proper care, person centred care, ensure about patient safety, healthcare
services, proper treatment, medication, support and help. These are some of the effective
facilities which can be effective for Liam (Opoka-Winiarska et. al., (2020)). There are some of
the effective facilities which are effective and allow to deliver quality care and treatment for the
patient better health. This is the one which can allow to provide and ensure about receiving
proper care and services which can support the patient physical and mentally for their better
health improvement (Mena‐Vázquez et. al., (2021)). There is need for the proper management
which can have high impact on the delivery of better care and allow to minimise the negative
health and provide some health benefits for better and improved health (Bollhalder et. al.,
(2020)).
While ensuring about the proper care and fulfil the need of required facilities for Liam,
there is need for the healthcare professionals and different healthcare premises which can have

high impact on delivery of health care services to Liam. This may include the healthcare
organisation like Accident and Emergency department, regular paediatric rheumatology (Al-
Mayouf, (2018)). These are the healthcare organisation which are considered for the Liam which
can allow to provide better healthcare services that can be effective and allow to deliver quality
services which can be effective for improvement in health. Dr. Jones, Registered nursing staff,
Liam paediatrician are the healthcare professionals who are having the information and attending
Liam health concern. Liam family member are also involved in his better support and care which
can allow Liam to get better improvement in health (Yuwen et. al., (2017)). These are effective
and allow to deliver the quality services to Liam for the better health concern. These are some of
the organisation and healthcare service providers who are having the effective role in delivery of
healthcare services and treatment to the Liam for his health improvement.
The radiographic hallmarks refer to primary osteoarthritis which includes the non-
uniform joint space loss. This includes the features of rheumatoid arthritis which are the joint
inflammation that includes the particular osteopenia, bone erosions, space loss, uniform joint
along with soft-tissues swellings.
After the recent asthma exacerbation pain has been experienced by Liam which needed to
be diagnosed and ensure about any uncertainty which can be effective and can cause negative
health impact on the Liam health. To ensure about the health concern Dr. Jones has referred
Liam for review to his regular paediatric rheumatologist within Sydney that is located 500km
away (Dipasquale et. al., (2019)). This was necessary for Dr. Jones who is the care provider of
Liam and have the responsibility to get ensure about delivery of proper care and treatment to
improve Liam current health. This referral needed to done for getting better visualisation of
health impact on Liam which needed to deliver better treatment and care plan role (Maschmeyer
et. al., (2021)).
Patients barrier to care goal
There is need to deliver proper care to the Liam which can be effective and allow to get
better and effective treatment for achieving the patient goal. In this, there are some of the barrier

implement in regular basis, avoid the care plan and other aspect which can restrict individual to
follow the self-management (Arman et. al., (2019)). These are effective and can create barrier for
Liam while achieving the care goal. In this, there is need to ensure about the better health and
should focus on the self-management which can allow to improve his health (Jabs, (2021)).
Patient is getting primary care while there was less chronic situation. Whereas, due to
getting chronic condition this leads to create situation to take secondary care for getting
healthcare services and treatment through the specialist for effective response towards achieving
better health.
These are some of the potential barrier which are effective and can create difficult in
delivering of care and treatment which can create barrier in achieving the care goals.
There is a situational complexity which may include the simple and technical complication
which may include the issue of getting worsen of asthma and getting degradation of arthritis
within patient that lead to impact the heath.

For proper treatment, there is need for healthcare profession who can share their service skill
and should ensure about the achieving the better health. This is the one in which registered nurse
have the role to deliver quality service and ensure about the Liam better health. This is the one
which need for different skill which should be possess by the Registered nursing staff. There is
high need to get ensure about the role of registered nurse in delivery of proper care and services.
In this, nursing staff should ensure about receiving and coordinating with healthcare professional
and family member which can be effective to ensure and support the patient better health. For,
this, there are some of the skills which are needed to use while delivering proper care and
services to Liam (Alfayadh, (2020)). There are some of the skills which can be effective and
allow to delivery better care as nursing practices which are going to discussed further.

In this, there is need to use the strategy of working together. This is the one which can be
effective in delivery of better care and support to the patient with JIA and asthma. This is the one
in through which nursing staff may get ensure about the patient healthcare needs and support
patient by implement the shared care plan of patient. Here, nursing staff should be contact with
Dr. Jones and his regular paediatric rheumatologist who have the information about Liam and
can help to share the required information that can be effective and help in implementing the
effective shared plan (Bradbury et. al., (2018)).

effective strategy that is effective and allow Liam to deliver quality care and share the healthcare

From the above discussion, it can be concluding that care plan is highly effective and should

of healthcare services. During this, there are some of the barrier like medical condition and self-
management which can have impact on the achieving the patient care goals. in this, there is
discussion about the nursing practice which required the effective skills like communication,
team working and networking which have role in getting proper care. There is also discussion
about the strategies which are effective to ensure about the effectively implementation of care
plan. This may include the programs, innovation and several organisations which can be
effective in ensuring about the implementation of shared care plan for delivery of better
healthcare treatment for better and improve health of patient.
