Reflection:
According to Midgley et al. (2013), nursing professionals must undertake reflection in order
to deliver effective care. Reflective practice has been identified to be helpful in improving the scope
of practice.
What?
In this case, I used the discroll method to reflect upon my clinical experience with the patient.
As this was my first clinical placement experience, I felt nervous and was not sure about my ability
to devise an appropriate care plan for the patient. I was able to accurately document the health issues
faced by the patient and with the guidance of my supervisor RN was able to plan effective care
strategies for the patient. However, as I was nervous I forgot to ask about the culture specific needs
of the patient and did not involve the patient actively in the decision making process. I did not
inform the patient about the details of her vital assessment. Therefore, I believe I was unable to
provide effective care to the patient. I personally feel that I must improve on these areas so as to
acquire increased level of patient satisfaction.
So what?
As a result of my clinical experience, I have identified two key areas where I need to improve
in order to acquire positive patient outcome. My clinical placement experience has taught me that I
need to be critically pay attention to all the aspects of patient care and at the same time also provide
care aligned to the NSQHS standard. Through my clinical experience I have understood the need to
implement the effective care model into practice. Working on these areas would help me with my
professional development and would also improve my confidence level while caring for another
patient with chronic illness.

Now what?
Therefore, based on my experience, I would brose resources online to learn more about the
NSQHS standards to render effective care to patients. On doing so, I will gain a better understanding
about the components that determine effective care. Also, in my next placement, I would make sure
that I involve the patient in the decision making process, pay attention to any culture specific
requirement and proactively convey information about any objective or subjective assessments. I
would make sure that during the care planning process, the patient is able to state their care specific
priorities. This would help me in creating a care plan aligned to the specific patient needs. Further, it
would also help in improving my communication ability and that would facilitate the creation of a
positive therapeutic relationship between me and the patient. Informing the patient about their
assessment results would help in maintaining a level of transparency in the communication and at the
same time will increase the trust between me and the patient. Also, while conducting a family
assessment I would not just prioritize the patient’s need but would also consider the needs of the
family members. Like in this case, I could have referred the patient for a family counselling. This
would have also addressed the psychological wellness need of Eva and Matt.
