The patient Mrs. Judy has been assessed

Person – Judy is a 29 years old woman, from
Ghana and English is her second language but
have been living in Canada for four years. She
is a single partner of two young children. Judy
work full time work at Tim Horton, and she is
the only care provider of the family. Judy has
gain 30 pounds of weight from fast food and
has difficulty in controlling her blood sugar.
Health – Judy has recently diagnosed with
type 2 diabetes. Due to her additional shifts in
Tim Hortons, she is unable to take good care
of her health. She has been eating more fast
foods to keep herself going. She has also
gained 30 pounds weight, while having
difficulties in her blood sugar control. She
also feels anxious nowadays, to fulfill all the
responsibilities. Her health history indicated
asthma in childhood, which has been
controlled well.
Environment – Living in Canada, Judy

misses her family, as she lacks family support
here. She can only skype her family once in a
week in Ghana.
Nursing – Nursing actions need to consider
her actual and potential concerns, mainly
focusing upon her current health issue of type
2 diabetes mellitus.
Social justice- Judy is a single parent and has
only one income which low to provide for her
family.
First Diagnosis Diagnosis is ineffective heath management
related to excessive demands and insufficient
social support as evidence by client statement
abilities to control her blood sugar level and
gained 30 pounds of weight.
Second Diagnosis Diagnosis is stress-overload related to
insufficient resources and excess stress as
evidence by client being a single parent with a
low income and her increasing feeling of
being anxiety.

Planning The plan is that the patient will review the

amount as well as the type of stressors in daily
living and the interventions will help the
patient to overcome the stress and live a
stress-free life.
First Nursing Intervention

Nurse will actively listen to the patient about
her life-stresses. Active listening will aid her
in feeling like she can discuss her stress
openly. Discussing openly about the stress is
also helpful in reducing it (Harvey, 2015).
Second Nursing Intervention

The second nursing intervention can be
instructing the patient about some stress
reduction technique such as yoga, deep-
breathing and it is also important to suggest
the patient for consulting a dietician in order
to control her diabetes with proper and
healthy diet. Stress reduction techniques will
help the patient in coping with the life stress;
the healthy diet will help to manage the
healthy weight as well. Deep-breathing
technique will also help in managing her
asthma (American Diabetes Association,
2018).
Third Nursing Intervention

The nurse will help Judy to discuss her stress
with her family members and to spend more
time with her young children. Discussing the
stress with loved ones helps in exchanging
feelings and emotions. Social activity and
spending time with children will help in

burning out the stress (Smith, 2014).
Evaluation Patient is able to identify the stress and
reasons behind that and she will begin to
journal her thoughts. The stress reduction
techniques are helping her to live a stress-free
life. A healthy diet intake is supporting to
control her weight. She is socially connected
and happy with her children.
