Introduction
The study is divided into two sections, in which the first section of a study will indicate
the medical history of the patient so that its severity can be compared with the admission criteria
and further steps can be taken place. In another section of the report importance of the case
management and disease management is explained. The main aim of the assignment is to get the
knowledge about the methods to handle cases or any disease in any clinic or hospital.
Case Study
According to the analysis conducted over the case study and by the examining the history
of the patient there were various possibilities found through which necessities were reflected for
admitting in hospital. The patient had the injury in the artery which occurs during the process of
catheterization so for the dissection of the artery it was important to admit the patient in hospital.
The catheterization of the patient was conducted 10 days before at that time the symptoms which
were explained by her were related to pain in her calf on walking 20 yards or less than that. Her
reports of the catheterization were reviewed by Dr. Smith. She was also get treated for the
cardiac transplantation before 10 years. The risk factors in which MI,CVA, death, infection,
nerve injury, loss in tissue and injury in bowel were included these all were discussed with the
patient as it is mandatory to explain all the risk factors as well before conducting operation and
after the approval of the patient hospital decides to perform operation.
Case Management
In the above case study the case manager identifies the past record of the patient and
according to the present condition the case was discussed with the Doctor. After analyzing

severity of patient which met with the requirements so that patient can get admitted at
hospital(Iliffe, Wilcock, Synek, Carboch, Hradcová, & Holmerová, 2019). The process play very
critical role as a communication line between patient and health providers.
Case Management Process
Case management process is helpful in achieving the best results with the satisfaction of
the patient as well as the resources of the health providers are used appropriately (Browne,
2019). Case Management Process is nine-stage procedure followed by the case managers to offer
quality of health services to the patients. It comprises of Screening, Assessing, Stratifying the
hazards, Planning, Implementing, and Following-up, Transitional care, Communicating Post
Transitional care and Evaluating. It helps the health providers to understand the concerns of
patient in proper manner. Through the application of management process managers can
determine various possible alternative to satisfy the need and health issues of the patients
Disease Management
The process, which is used to manage the chronic diseases and to improve the quality of
life, is known as disease management. In healthcare the disease management program is very
dominant for improvement of the medical treatment by considering the long term goals (Holland,
Watson, & Glaspole, 2019). The chronic disease requires long term treatment in which the
evaluation of patient is performed regularly. Disease management program manages all the
required information of the patient so that the health experts can evaluate the condition of the
patient. As similar in the case study where the information of the patient was available related to
the therapy provided 10 years ago. On the basis of which the patient was admitted and advised
for the operation (Patel, 2019).

It is a population identification process along with the probability of enrolling patients.
The main objective of following the this process is to evaluate the health care system and its
usage to determine the effective of the program over the clients or patients

as indicated.

Conclusion
The assignment concludes about two important process in the health care which entitles
as case management process and disease management process. The report is prepared through
the case study in which situation of the patient is explained with her history. In the assignment
through a scenario of the patient the importance of both of these processes is explained briefly.
