INTRODUCTION
Nursing care management functions is important in order to get appropriate result's of
the selected treatment or cure intervention. Nurses have to accomplish and manage various
health care practises suggested by the doctors as a part of treatment. They have to perform their
activities and functions on the basis of health care legislation and evidence based practices. In
the case of chronic disease management the most important thing along with the treatment is
health care education of the patient according to which they can take the safe decisions. In this
report various roles and activities of the registered nurse is going to be consider such as the
strategies and assessments which should b done by the nurses for proper care (Heitkam, 2019).
Evaluation of the effectiveness and outcomes of the strategies and assessment use by nurse, that
how it can contribute to the best out comes.
MAIN BODY
Nursing care actives
Registered nurses are the important part of the health care occupation for various good
reason, they have to give medical care to the patient along with the physical and emotional
support. Providing education to the patient is another important function of nurses which has to
be done in order to manage the chronic health situation (Bakker, and et. al., 2018). Registered
nurses have to work in various place in prospect to manage their patient such as in the hospital,
doctor office, home care service and in clinics.
Nursing care role-
In context to the given chronic health care case where Mathew is the patient for which
home nursing care is required. Mathew is suffering from the type 2 diabetes and have fear of
losing his limb's due to the chances of peripheral neuropathy, and in order to manage that hi
want to loss his Wight. As Registered nurse I have to use the appropriate health care strategies
and assessment. First I have to asses the patient condition and record patient medical histories
along with the symptom's. Then on the basis of medical test and analysis, doctors instruction as
a nurse I have to observe the patient regularly and record that information. Patient medical
treatment and medication prescription have to be take from the doctor. Then on the basis of
patient test result or situation an effective care plan have to be develop by concerning it with his
family and himself. I have to take care of the regular treatment and hospital visit in order to

evaluate the care result's and to collect more guidelines of doctors. I have to educate the patient
about health care precautions along with this his family has to be educated for effective holistic
care procedure, in which they can contribute for the betterment of patient. For Mathew care and
management holistic care plan is selected which involves the different home care and hospital
care services.
Holistic care
Holistic care is one of the most effective health care approach involves the care of whole
personal different from the normal care services which just focus on the diagnosis and treatment
of the issues (Lindley, and Keim-Malpass, 2017). Holistic care is involving person centred practising
with includes, physical care services, social care, medical care, personal care, emotional care
support in order to help them for effective cure or control of the problem. Mathew case holistic
care is important because hi is facing the various other problems such mental illness due to the
adverse impact of type 2 diabetes, Mathew needs the lifestyle management care, medical care,
along with the emotional support which can be possible by the aid of holistic care.
Holistic care plan and intervention for Mathew
In order to provide the effective holistic care services that is important to influence the
family and patient too. For participating in the care process by their interest and support.
Mathew sister is having 3 childes with whom he enjoy a lot so they can be a part of the holistic
care in order to provide emotional support, Mathew is over weight and he has to loose that in
order to make his heath better and to remove the risk of Peripheral Edema. Below is the

application of National strategic framework. There are the three major objectives selected by this
framework in order to control chronic disease from nation. First is evaluation and focus on the
prevention strategies for management of healthier Australian (Hutchison, and et. al.,2017). Second
objective is to provide the best care services to support people facing chronic health care
problem in order to provide them quality of life. Third objectives is to target the population for
management and improvement. Vision behind these objectives is to achieve healthier living and
management of the people who are facing their chronic disease (Gillespie, and et. al., 2019).
Nurses role and patient self management approach is the part of framework. Nurses have
to conduct various patient management role such as, assessment of the patient health, education
of the patient regarding self management approach through which contribution of Mathew can
work effective to control health problems (Toyama, and et. al., 2017). Risk assessment, timely
detection, as a nurse I have to effectively perform the care practices on the basis of National

strategic framework with in the aid of effective, doctor, patient, family and government
partnership in context to management of the chronic and reduction of the death rate and
unhealthy life (Nazarko, and Thorne, 2020). As nurse its a responsibilities to evaluate the chronic
disease care policies and then plan to provide the effective care according to polices and
legislation. Self management approach is the important strategy of chronic disease care
farmworker, so this approach is involving analysis of the self condition, follow the care plan
instructions, monitoring the symptom's, following the healthy life style.
Evaluation of the health outcome
In context to the case of Mathew holistic care service plan is selected, with in the
application of National strategic framework. It helps to effectively plan for the holistic care
practices such as it involves the structure which guide to set the appropriate plan with in the
goal of optimum outcomes. Such as in this case most important and evidence based interventions
are selected to achieve the weight loss, so the health management outcome is lower body weight
which can remove the various issues of higher body weight in context to type 2 diabetes. Then
another selected outcome is healthy life of the patient which can help him to live quality of life.
Mathew is facing the problem in the movement of limbs, which make him mentality ill, so in
context to this goal for the outcome is improvement in the limbs movements and positive mental
health. So with in the combination all selected objectives and outcomes one of the major
outcome should be better quality of life (Chen, and et. al.,2018).

From the above study it has been concluded that the role of nurses in the case of chronic
care, is very important and essential in order to control the impact of chronic disease and to help
the patient for living the quality of life. Nurses have to take the right care action according to the
care plan which they develop for holistic care. Practices should be evidence based and effective.
In the holistic care plan nurses have to involve the activities and practices by informing the
patient and his family. As a registered nurse that is the responsibility to manage the patient
requirement and health care every time. Holistic care is the health care service which involves
the person centred care with in the collective activities such as medical assessment and treatment
, personal care and patient life style management in order improve health. Some home care
interventions and emotional support is provide to the patient under holistic care. In order achieve
the goal of the quality life. That is the legislation of UK nursing care services to implement the
evidence based services and involve or inform the patient for care plan procedures in order to
encourage then for make their own choice.
