This report presents a comprehensive analysis of the Zhang family case study, focusing on the application of the nursing process in a family-centered care context. The student identifies the challenges faced by the family, including the parents' workload and its impact on their child's behavior and the grandparents. The report establishes a nursing goal to improve the family's quality of life through effective child care. It details two nursing interventions: assisting parents in providing effective care and teaching them time management skills. Problem identified: Recognition of challenges/issues faced by the family
a) From the review of the case study, the problem identified in the family is lack of
sufficient time that can be delivered for the care of child due to huge workload of both
Zhang and Daiyu. Due to workload, the Zhang and Daiyu are not able to give quality
time to their child and grandparents, which is negatively impacting their child’s
behaviour and causing health issue in parents. Zhang has to do overtime to earn more so
that he could pay the medical bills for his father. This has forced Daiyu to work full time
and join hospital earlier than the planned dates. This problem is causing prevailing of
unhealthy behaviour of their child, which is affecting their development (Jia et al., 2016).
Even this may cause dispute in the family as parents of Zhang are annoying due to the
tantrum of the child.
b) The main nursing goal for the family will be to improve the quality of life of Zhang and
Daiyu by assisting them in taking effective care of their child. It is reported from the
study of Birmaher and Brent (2015) that tantrums and clinging behaviour of child can be
seen because of less attention and time given by the parents. This impact the psychology
of the child, which can pose negative effect on the quality of life of parents and family. It
is reported that disputes among the parents can happen due to inability to take of care,
which also has an adverse effect on their life and affect their dignity (Sitimin et al.,
2017). Therefore, the central goal of nursing will be to assist them by giving proper
intervention for taking practical care of their child, so that it can improve their quality of
life and provide mental stability.
c) Nursing care interventions: The two nursing care interventions that would be taken to
improve the quality of life of the parents would be

 Nurse would assist parents in taking effective care of their child: Nurse will provide
effective teaching to the parents regarding ways to take active care of their child. It is
noted in the case study that, both parents were not able to provide sufficient time to their
child due to their workload and financial issues. The nurse would narrate how to train the
child for toilet sitting, ways by which Lucy can be kept calm and relax, teaching the
parents to allow child to make their personal choice and encourage them to use words or
communicate with gestures. It is noted that Lucy showed tantrums at nights when Jin and
Daiyu leave for their work. Hence, nurse will train the parents to make the child engage
in activities she likes so that she would not ponder on their absence. It is seen in the study
of Ordway et al. (2015), where the author has discussed the child activities. The author
has highlighted that child like to do creative activities which help in diverting their mind.
A nurse will provide environment that will be free from distractions, as this will help in
dealing with the external stimulation will be improved of child. It is seen in the study of
Smith et al. (2015) that author has given idea about the importance of interaction with
their child, which helps in healthy development of child. Nurse will provide teaching of
positive parenting techniques to both grandparents and parents like they must positively
respond to their children and assist a child in exploring new things.
 Nurse would teach the parents about time management- It is reported from the study of
Beck et al. (2015) where the author has explained about the importance of time
management in the life of parents that would have impact on their life and family. Patient
centred care will be provided by the nurse, which will include the family in decision
making (Liang et al., 2019). Giving ample amount of time is necessary for the healthy
development of the child and wellbeing of the family. It is seen in the case study that

parents due to heavy workload were not able to manage their time, which was creating a
negative effect on the child and family. The grandparents were withdrawing and snapping
at the children due to their tantrum. Hence, for the nurse, it is essential to address the time
management skill for a better quality of life. A nurse will teach the parents to prioritize
their need, then work accordingly. Nurse would help the parents to make list of priority
things and ways to organize the task in such a way that effective time will be provided by
one of the parents to the family (Pierre & Conley, 2017). Nurse will educate the parents
to communicate about the issue so that time management can be done per the priority.
Nurse would assist the parents in making plan that will allocate the individual task as per
time so that both of the parents to not invest same time to work.
d) Evaluation of nursing care intervention
To evaluate the nursing intervention, which is planned for the goal of improving quality
of life, the nurse will make a weekly visit to the family and observe the effect of teaching that
has been provided. The nurse will evaluate the behaviour of a child by interacting and playing
with them and investigating with the parents and grandparents (Weldam et al., 2017). The nurse
will ask the parents to assist the child in the toilet, and the behaviour of the child will be noted.
Nurse will also note whether the child is showing tantrums or showing clinging behaviour.
In order to evaluate the intervention for the time management, nurse would investigate
the parent about their daily schedule and number of hours they stay at home with the child. It
will also be evaluated by assessing the behaviour of child, because if effective time is being
delivered by the parents, there must be healthy change in the behaviour of child (Östlund et al.,
2016). All the changes will be recorded by the nurse, and if there is no improvement seen in the

life of the parents and family, nurse can make desired amendment in the intervention, or it would
be extended for more period of time.
Part 2. Impact of policy on children and families
Vaccination is the most important method of protecting the child from various type of
infection and disease (Brotherton et al., 2018). In Australia, there is a strict law that has been
introduced in the legislation which bans the enrolment of the unvaccinated children in the kinder
garden and other preschools. The Australian Government Department of Health and
Australian Technical Group of Immunisation has developed implementation of the policies
concerning the immunisation in the name of National Immunisation Program (Gidding et al.,
2017). The law state that all children need to be vaccinated before being admitted to school. The
legislation of Australia discusses that a fine of 25,000 Australian dollars will be imposed on the
parents for not vaccinating their children and admitting the children into the school (Baxter,
Clothier & Perrett, 2018). The law of Australia gives full liberty to the child care centre to
exempt the children who are not vaccinated. Concerning the area of distress for Daiyu regarding
vaccination policy which has instructed him to keep their children vaccinated till the date they
attends school, nurse response is given to address the reason behind it. It is seen from the case
study that Daiyu has not vaccinated their child due to the fear that, it will have negative impact
on their health. Additionally, he is in the state of dilemma that their child would not get
admission due to vaccination policy in Australia and in addition. After listening to Daiyu, it is
the responsibility of nurse to educate Daiyu about the importance of vaccination. Nurse will give
quality teaching about the role of vaccination in order to eliminate the misconception regarding
vaccination (Phillips et al., 2017). It is important to make Daiyu aware about vaccination that it
protect the children from acquiring lethal disease like Chickenpox, Diphtheria, tetanus, Hepatitis

A, Influenza and many other. It boost the immune system of child against fighting with infection.
Nurse will make him understand that it does not have any adverse effect on the health of the
child. Further, nurse will also make Daiyu aware about the national immunisation program that
he will be penalized for admitting the child without vaccination and it may lead to some legal
action against him for his such activity. The nurse also inform him that preschool checks the
documentation of vaccination before taking admission, and they have the right to exempt their
child from being admitted in the school. It is the role of nurse to provide quality knowledge
about the concern area of parents with the objective to provide effective care plan (Ryan et al.,
2018). Nurse would advise him to get their children vaccinated in order to protect themselves
from legal action and moreover it will boost the immune system of their children.
