Introduction
Teaching and learning the new changes and development in the profession of nursing is
essential, nurses dealing the patient health care need to study regular about the health issues,
cuss of the dieses, treatment for the issues and other relevant information facilitating to
understand and prescribe the client about the health issues and care. This report is a detailed
analysis of role of nursing health care plan, in health care practice of a patient. Patient
selected for the health plan is a female with the Tracheostomy, and will make a health plan
for its health care activities (Joyce, Murphy, Bennett, Sweeney, & MacLoughlin, 2019).
This report will review some books and articles discussing the importance of the health plan
for a patient with Tracheostomy, to make identification of the actions need improvement or
are improving in the patient, this report will analyse the importance of role played by a
patient’s interest. An interested patient for health plan facilitate nurses to examine the causes
and actual health conditions of their patient and this also help them to make further planning
to support better health and living in the future. Functional health plan will be helpful to
examine the physical need and requirements in a patient with Tracheostomy and diagnose the
impact of the medicines on patient as given to her by the doctors (Edelman, Mandle, &
Kudzma, 2017).
Patient’s brief history
Alexi, a 42 year’s female with Tracheostomy is client. She had a Tracheostomy surgery 4
months ago, with the expert’s surgeon of Tracheostomy. She had an asthmatic issue from her
childhood, after the age of 38 problems became severe and caused secretions and issues to the
clear breathing. After a successful surgery and medical prescriptions for the 3 months, she is
under diagnosing to examining the impact of medicine and surgery with the help of the

Gordon’s Functional Health Patterns. This surgery was done to clear the breathing, hence this
report will use the literatures review to analyse the importance of health plan in management
of patient health (Mahnaz, Shahin, & Shahdoust, 2019).

Gordon’s Functional Health Patterns for the Tracheostomy patient is a famous method
developed by Marjory Gordon, helpful for the nurses to get detailed examination of the

According to (Karaca 2016), functional health assessment is a base to collect and manage
data about the patient. This model uses a systematic determination and collection of the
factors affecting the health issues and causes of the dieses in a patient. A systematic tabular
representation allows the nurses to compare the previously collected data with the present
situation to analyse the impact of the medicine, prescription and other activities on the health
of person (Antonetti, et al., 2018). Thinking, perception, motivation, and religious beliefs
affect the healthcare determination in a patient. Patients with breathing issues require
treatment of Tracheostomy surgery but lack of systematic review about the patient’s
information can lead issues in the health care practices. Functional Health care model allows
nurses to discuss about family history of disease, personal beliefs, and other important factors
to make the task of operational activities easier and perfect for the further examination
(Sexton, 2017).
A surgical procedure performed to open a way through the neck for proper breathing and
access of breathing activities to the breathing tube; it is a surgically created breathing tube.
According to the (Santos, Harpor, Gandy and Buchanan, 2018), a Tracheostomy patient need

enough skilled staff with multidisciplinary tracheostomy staff, specialised in the preparation
of functional health plan nursing staff firstly need to analyse the all eleven functions related
to the patient health care history, case, previous issues, nature, habits diet and working
activities (Arkowitz, Miller, & Rollnick, 2015). The doctors in critical health care situations
suggest tracheostomy surgery, and nurses, surgeon with the help each other mange
operational activities. Functional health plan used after surgery helps doctors to understand
the success of surgery in a patient (Santos, Harper, Gandy, & Buchanan, 2018).
According to (Patton, 2019), creation of surgical airway is a common procedure to replacing
the inhaling tube by connecting an artificial tube to the lungs for better experience of inhaling
and curing the breathing issues. This surgery is usually performed in the critical situation as
acknowledged by the doctors, hence using the teaching and learning model regarding the
issues, surgery, outcomes on the patients’ health after surgery and to measure the changes
with the previous condition require proper assessment of the health and habitual activities of
the patient. This learning activities are recorded in the function health model motivate patient
by comparing the present positive or negative situations with the previous situation and then
it motivate a patient to adapt changes in their habit to live healthy (Patton, 2019).
According to (BSN, 2016), based on a survey in the Taiwan’s traditional culture most of the
people are not trust worthy to the adaption of surgery as tracheostomy, nurses conducting the
functional health assessment plan need to use the Gordon’s health modal. This model of
assessment is very helpful to examine the eating, daily routine and religious beliefs of a
patient. This model allows the nurses to understand the personality and behaviour of a patient
and to determine the conversation round to motivate the people about the suitability of the
surgery. All factors of the model include small relevant aspect of human nature, proper
understanding and faith among the nurses and patient lead the operational activities flawless
and mistake free (Hsu & Chen, 2018).

issues (Johnstone, 2018). Her physical structure was thin and she had junk food addiction as
she has modern lifestyle that cause issues and increased in the health issues breathing
problems. Alexi found the difficulties to adapt the changes related to the food and cold
beverage consumption, she had allergies with the cold atmosphere and medical diagnosis
examined that cold weather affect the issues very seriously. Sinus infections in the nasal way
also lead the difficult breathing and other issues in the health care of Alexi (Choi & Emily,
2016).
Functional health survey filled by the practising nurses is reflecting the changes adapted by
the Alexi to care of the breathing issues and sinus infections. This model of systematic data

pressure of medical activities.
Resources and opportunities for the client
Resources adapted in the systematic review of health function in the Alexi are related to the
family questionnaire round and routine activities information collection by the direct
interviews conducted with the help of open-ended questionnaires. After the surgery, Alexi is
supposed to face several health issues with the changes in her body.
Opportunities with the Alexi are relevant to the body structure, healthy eating habits, and
adaption of the proper health care activities and medicine required keeping the issue of
breathing solved in her further life span. As discussed in the findings of (Edelman and
Kudzma, 2018), medical sector has adapted many technological, social and systematic
reviewing methods these methods will help the nurses to examine the issues and impact of
these issues on the health of patients (Geyer, 2016).
Findings and learning
Gordon’s health functions deliver a complete nursing assessment of patients in the duration
of the patient’s health care services (Dickinson, Taylor, & Anton, 2018). The categories of
the systematic review of the patients’ perception, health management, sleep and rest, roles
and relationship of the doctors and patient nursing relationship and assessment to measure the
stress tolerance in the patient. This model is very helpful but requires very effective interview
techniques with a well-structured questionnaire and motivational interviews to acquire the
proper knowledge and data about the learning of patients’ health factors.

This report has detailed discussion about use of systematic health assessment tool introduced
by Marjory Gordon in 1987. This reflects the use of communication as the method of
motivation and adaption of change in a patient facing the issues related to the health. Patient
in this report facing the breathing issue was given tracheostomy surgery; nurses in this report
had adapted the systematic collection methods of Functional Health model to analyse the
difference in the health condition of the Alexi after the surgery. This method use personal
perception, habits, routine activities, and other relevant features in the patients’ habits and
behaviours. Hence, use of Gordon’s Functional Health systematic review is helpful to
manage the activities for the nurses these activities help time to plan their action for the next
medicine or prescription in the nurses.
