Am delighted this evening to present you some of the very intimate key elements that play a
major role in the general conduct of this private institution. It is in the interest of the government,
the public and individuals that we should provide exemptional services in the entire course of our
time in the nursing home. It therefore calls for harmony, openness and strait forwardness in
whatever we do keeping in mind that it is not just an obligation.
It is important to recognize the fact that Nursing homes are critical health partitioning intexts that
require adequate attention, knowledge and evidence based practices to meet the expectation of
the government policies and promote safe community in coordination of care. Similarly, there is
a great need that continuum of care be applied constantly in our daily lives. Continuum of care in
nursing homes is usually from the independent living the skilled care in nursing. The services
that are progressively offered by the concerned health body and practitioner teams allow the
residents in a nursing home to get whatever level of care they would want to get and whenever
they need.
As I mentioned earlier, the government is interested in the affairs that goes o in the nursing
homes. regarding this, the government has formulated policies that relate to the health and safety
of the nursing home. Ultimately understanding that coordination of care is practice program that
aim at supporting implementation of the government's policies makes us aware that the
government policies have an impact on the health of citizens. This could be through influence of
the social determinants of health. Changes in social policy like education, housing and even
security may cause a shift in public health specialty. Some of the policies affecting nursing
homes include: Medicare-insurance programs that covers health acre for the elderly and those
with end stage renal disease, Medicaid- Public insurance programs covering health care for the
poor, nursing home reform act and Health insurance portability and Accountability Act.

In all these, it is essential that we understand some of the national and local policy provision that
raise questions for care coordination. One the dilemma is the affordable care act care- the Obama
care had a lot of issues regarding the insurance cover. Millions of people were not able to renew
their existing, non-fictional and non-conforming insurance policies. The other one is that
Uninsured residents have no easy access to the private offices of the physicians. They seek care
from the safety net providers because they are uninsured. Lastly, the policies provide that there
should be insurance policy for the elderly unfortunately there are elderly people who are still not
insured and suffer relentlessly in the nursing homes.
As well, these policies have implications in the lives of the patients and the health practitioners
which is also essential to understand. One of the implications is that Medicare has helped a great
deal in providing the necessary attention that the disabled and those with end stage renal disease.
It has positively contributed to the wellbeing of the beneficiaries. Another implication is that
Medicaid has helped in taking care of the elderly especially those who are beyond a productive
age. However, it is also unfortunate that those without insurance policy have no option when
they are denied access to the private physicians.
Some of the impact of the code of ethics for nurses on the coordination and continuum includes:
enhancement professionality among nurses in the nursing homes, that nurses can treat patients in
the nursing home with absolute dignity, keep their information confidential and walk with them
throughout the journey of illnesses. Lastly, Code of ethics has enhanced proper values, beliefs
and practices among the nurses so that the nurses provide comprehensive and holistic care that
promotes health care in nursing homes. The consequential factors that contribute to health, heath
disparities ad access to services include but not limited to Development of disease or an illness,
Ability to get ad utilize healthcare and Health and healthcare outcomes.

Key ethical and policy issues that has a direct impact on the coordination and continuum of care
include Professional autonomy and loyalty, involvement boundaries and that that sometimes, the
health professionals choose to go against the code of ethics at the expense of the ailing
individuals.
