Introduction:
The role of health professionals is important for individual in order to remain healthy and
maintain well-being. There are different health professionals that provide effective service to the
people for example, nurses, doctors, pharmacist, dietician, physiotherapist and many others and
they play different role in order to provide well-being to the individual (Warrington et al., 2012).
The purpose of the assignment is to understand the role of multidisciplinary team, reflect on the
contribution of two members of multidisciplinary team and prepare a report based on the
reflection. The report will discuss briefly about the role of different health professional in
providing effective care and maintaining well-being and outline the importance of effective
communication in patient centred care.
Reflection:
Due to recognition of diabetes I was suggested to have proper diet and take effective
medication in order to resolve my health issue. In this regard I have visited to a dietician and a
pharmacist to in order meet my health needs. Beside the help of the doctors and nurse in the
hospital I was satisfied by the contribution of the Pharmacist and the dietician in order to help me
in managing my illness. The pharmacist has provided effective medication in order to reduce the
severe effects of diabetes and the dietician has provided an effective diet chart that has helped me
to reduce my weight, manage blood glucose level and maintain well-being. Through their
contribution the health professionals have supported me in my health concern in an effective
manner. Their effective communication skill has helped me to understand the treatment process.
They communicate politely that has helped me reduce my pressure thus I have participated in the

process effectively. The pharmacist has written minor details in a paper to make me understand
the dosage of the medicines and the dietician has provided a diet chart to maintain regular food
habit and adapt healthy eating. In addition the pharmacist has provided information regarding the
Diabetes Medication Assistance Service or DMAS that has helped me to improve self-
management. Such effort of the health professionals has helped me to understand what they were
saying. During the interaction both of the health professionals has allowed me to speak for
myself. I have shared all my health problems and they provide full attention to my conversation.
I have asked so many questions regarding my health and the treatment process in order to
understand my health condition and take proper decision about the treatment. Their support has
helped me clear all doubts and take proper decision. They have asked me about my consent
regarding the treatment process. In this way the health professionals have provided respect to my
dignity and autonomy. Through the interaction I was involved in making decision regarding my
health. The session has helped me to understand the value of different health professionals and I
was happy to have such experience. The effort of the health professionals has helped me to
manage the illness and improve my health in an effective manner.

Role of multidisciplinary team:
The multidisciplinary team plays an important role in order provide effective treatment to
prevent and cure illness. It is the combined effort of the team and their coordination that helps to
remain healthy and maintain well-being in an effective manner. The member of the
multidisciplinary team such as the pharmacist and the dietician play a vital role in order to

manage diabetes. Their knowledge and skills help a patient to maintain their well-being while
suffering from diabetes (Warrington et al., 2012).
The type 1 diabetes is an autoimmune disease and in this case the body reacts to gluten
that damages the gut lining and creates difficulties in food absorption and type 2 diabetes is
mainly caused due to high glucose level in the blood. Thus it is important to have proper diet in
order to maintain the blood glucose level and adapt healthy eating habits. The dietician provides
effective diet chart to improve the food habit of the patient in order to resolve the issue. In this
regards the dietician provides effective diet chart that includes adequate amount of nutrients for
the patient and restrict the patient to have food that could increase blood glucose level, thus help
the patient to manage the diabetes and remain healthy (Deed et al., 2015).
Proper medication is also important in order to manage diabetes. In this regard the
pharmacist plays an important role. It is the responsibility of the pharmacist to help the patient in
understanding the medication process. In addition the pharmacist provides Diabetes Medication
Assistance Service or DMAS which is a self-management support provided by the community
pharmacy to help the patient in managing medication. Such support helps the patients to control
hyperglycaemia and improve their health condition (Hughes et al., 2017). The effective
contribution of the pharmacist helps to achieve positive outcomes.
All the members of the multidisciplinary team play important role in order to provide
effective care, but there are some difference in their responsibilities. In order to compare and
contrast the role of different member of multidisciplinary team it can be elaborated that the
registered nurse also provides effective service to the people with need, but the difference is it is
the responsibility of the nurse to provide time to time medicines, take care of the food habit and

help in risk assessment of the patient in health care centre. Whereas, the role of pharmacist is to
provide effective medication and decide the proper dosage for individual and dietician helps the
patient to maintain proper food habit which could be beneficial to maintain healthy life. The
nurse provide care according to the guidance of the pharmacist regarding the medication and
follow the guidance of the dietician in order to take care of the food habit of the patient.
However, role of each health professional is equally important. It is their combined effort that
helps to get rid of illness and maintain well-being (Warrington et al., 2012).
Patient centred care and communication:
Patient-centred care is defined as the care provided by the health care service considering
the individual needs of the patients, their life circumstances and care that is respectful to the
value, dignity and decisions of the patient regarding the treatment process. Communication skill
of the health professionals plays an important role in order to provide effective patient-centred
care. The guidelines related to patient-centred care provides effective ways to start conversation
with a patient (Bell, 2014). The useful conversation helps to understand the problem of the
patient in depth, thus the health professional could introduce appropriate strategies and effective
treatment in order to resolve the health issue (Bachmeier, 2011).
The health professionals in this case have used effective communication skill in order to
provide patient centred care. They have asked about the health problems which reflect their
concern regarding the health of the patient. Such conversation has helped the health professionals
to understand the health issue and central need of the patient thus helped to introduce effective
strategies in order to mitigate the health issues. The support of the health professional has helped
to understand the treatment process thus make it easy to get consent of the patient regarding the

treatment process. The pharmacist has written minor details regarding the dosage and timing of
medication which has helped to understand the medication process in a better way. The dietician
has provided a diet chart and has provided information about the importance of proper diet in
case of managing diabetes in an effective manner. Such effort has encouraged the patient to
contribute in the treatment process. The health professional has provided respect to the dignity
and autonomy of the patient regarding the decision making power which is an important feature
of patient centred care (Bell, 2014). Such skills of the health professional have helped them to
gain the trust of the patient and the patient has participated in the interaction in an effective
manner. Their support has helped the patient to manage the illness effectively.
Conclusion:
Thus it can be said that, the role of each health professional is important in order to
provide effective care service to the patients. In this regards effective communication skill is
important to make the treatment process easy to understand and encourage the patient to
participate effectively. It is combined effort of the multidisciplinary team that helps a patient to
be recovered from the illness. The role of dietician and the pharmacist is as important as other
health professionals in managing diabetes. It is their effort that helps the patient to manage
diabetes and maintain well-being in an effective manner.
