This is to inform Rosaria about the hysterical condition of Maria that has in turn injured her
father and he is bleeding very badly. Also, due to his breathing complications, the entire situation
seems to be very critical and Mr Italiano has been taken into the ambulance by the officers. In
such state, though I have managed to reach Rosaria on call, it is important to handle the hysteric
condition of her mother who is getting uncontrollable. I will thereby ask Rosaria to assist me in
doing so by firstly scrutinising her current circumstances. In case, she is not full of activity and
can come back home. I will refer to convey her the entire situation.

as specified below with the help of an ISOBAR-
I stand for identify- I am representing an enrolled nurse working under your guidance
with a male patient named Corrado Italiaono, aged 84 years and his 64 years old wife
named Maria.
S stand for situation- The current situation is about Mr Italiaono where he is dealing with
a severe injury and continuous bleeding due to an attack done by his hysteric wife by
throwing a saucepan at him.
O stands observation- Due to some previous breathing related issues, he has been
assessed on the basis of both his bleeding and inhalation where the breathing is
controllable right now. Although, he has been admitted to ER to take care of his wound
and manage the same through suturing.
B stands for background- This is on referring to some of his past medical histories where
he has been discharged recently after getting admitted for 10 days due to his COPD
related issues. This is done after a strict prescription of some specific inhalers and
nebulizers to be used as instructed by the physician.
A stands for assessment- This is on assessing the present situation of Mr Italiano where
he has been injured by his wife and is bleeding continuously with a normal breathing rate.
Followed by which, he has been taken to the emergency department by the ambulance
officers to immediately treat his wound.
R stands for recommendations- This is to recommend effective measures for the patient
who has been just discharged from the hospital before 10 days on the agreed clauses of

enrolling for COPD rehabilitation program. It is therefore suggested to carry out a
complete evaluation of his existent situation and provide accordant treatment and
medications. It is with a special consideration of avoiding any atrocious consequences of
his breathing disorder.
