The three key determinants of health include physical environment, individual behavior and the
socioeconomic factors (Nash et al., 2016). At the place of my work, the most common
determinants of health are individual behaviors. Majority of the clients that I have had to handle
are drug addicts. This has really affected their health (www.coloradohealthinstitute.org, 2017).
However, there are strategies that can be used to limit such behaviors and improve the population
health. The triple Aim model is an example. Its objective is assist healthcare providers meet the
needs of their patients.
Discussion Point 2: For some example and measure, the book lists "reducing ER visits" (Chap
14, p 288), propose the macro-, meso- and micro level goals for the measure with description of
your rationale
For my assignment, I chose absenteeism since this is one of the greatest challenges at my current
place of practice. This is common among diabetic patients.
Meso-level-there should be a state mandate or rather policies that address absentees. The policy
should be clear to leaders of the hospitals on what they should do in regards to the mentioned
problem.
Macro-level-The clinic should set up its own policies that address absenteeism.

Micro-level-There should be a specialized individual to educate the patients on the policies of
absenteeism and their general health effects.

I really like your introduction since you have set the stage by introducing the three major
functions of measurement in health. You have stated improvement, accountability and finally
research as the purposes of measurement in population health. You have gone a step further and

finally income and education. I like this determinants since they are basic to everyone. You have
gone some miles and provided examples of the effects of low standards of education as a
determinant of health in Pueblo while providing contrast to the healthy and wealthy residents of
Versus Douglas due to the high levels of education that they have attained. This real examples
are quite good as they give the readers easy time to understand your information. You have also
provided the Triple Aim Model and Chronic Care Model as tools that nurses can use to design
the type of care needed by patients.
On point two, you have clearly proposed the necessary meso, macro and micro level
goals for measures and you have as well described their rationale. In conclusion, you have made

I really like your introduction. This is because you have gone straight to the point and
introduced the determinants of health of your choice. You selected smoking, access to sugar and
lack of exercise as your determinants of health. This is very noble since this are common
amongst different people. I like that part where you have said that negative behavioral patterns in
relation to mentioned determinants of health contribute to premature deaths. You have also
linked and provided a reference of the Nash et al to show that this is not your original idea and it
is good when writing a scientific assignment. I also like that part where you have provided the
measures of care experience Nash et al (2016). This are very important as they assist the nurses
to evaluate the type of care they accord different patients. You mentioned six of them in your
assignment that include effectiveness, safe, patient centered, timely, equitable and efficient.
These measures are very vital and all the nurses should take into account during their regular
practice. I like your conclusion since you have related costs of healthcare and behavioral aspects.
Negative behaviors are bound to increase costs of healthcare and you have provided increased
smoking and consumption of excess sugar as an example. Healthy behaviors on the other hand
reduce costs of healthcare and you have linked this with regular exercise. In general, I like your
