In order to provide safe and responsive care to the patients, registered nurse must acquire skills and involve themselves in
continuous development process. The continuous professional development and education assist nurses to improve critical thinking
and decision making which in turn facilitate the professional practice. The purpose of this essay is to develop learning plan for two
learning needs identified in part A such as standard 1.6 and standard 2.2. The standard 1.6 stated that it is responsibility of the nurse
to conduct accurate and systematic assessment with efficient communication skills. To address this learning need, the involvement
in various activities such as involve in ISBAR of the patient history and vital signs in clinical practice, assessment of the patients
in clinical practice in presence of supervisors, reading journals and listening to ted talk. While I gathered knowledge in
documentation and comprehensive assessment, I was not confident enough to document every cues. These activities will guide me to
develop knowledge regarding documentation and comprehensive assessment of the patient. Hence, with the assistance of these
activities , I would be able to gain knowledge and confidence which will further assist me to provide care which ensures patients’
safety.
On the other hand, the second learning need for which I would require to involve myself in learning plan is effective
communication skills according to standard 2.2 of NMBA. It stated that it is necessary to build therapeutic relationship with patients

since it improve patients satisfaction, empower patients and improve patient participation. It can be built by involving myself
training and workshops for active listening, non-judgemental approach. I will ask patients questions, speak at measured pace and take
time to responded with empathy in presence of supervisor. I will also gather knowledge through journals and ted talks. The detailed

For evaluating the learning experience, Rolf’s reflection model will be used.
What?
My learning need 1 was to increase knowledge regarding the proper documentation, improve decisions making and planning
and evaluation process. During the process of transition to professional practice, I was not confident enough regarding proper
documentation which affected my nursing practice since I failed to document various crucial cues that impacted practice. I gathered
the understanding that without proper documentation of comprehensive assessment, the quality of care can be compromised. The
potential errors associated with a lack of proper documentation and maintenance are medication errors and other clinical errors which
can subject patients into potential harm and questioned patient safety. In this context, becoming confident and considering each point
of vital assessment can facilitate decision making, planning, and evaluation.
So what?
Lack of confidence, as well as knowledge along with experience, had highlighted that the need of involving in training and workshops
along with the evidence-based knowledge in the area of safe documentation of assessment, planning, and evaluation. Hence, I involve
myself in activities that assisted me to gather information and improve my confidence. I believe that after involving myself in the
learning activities, I have gathered an in-depth insight into nursing values and responsibilities which will further help me in the future
to empathize with patients and empower them and respect their dignity. The first activity was clinical placement where I involved in
documentation of vital signs. The values I have gathered is that it is critical to conduct appropriate assessments and immediately
