Identification of quality improvement activity and their important to improve patient safety
Conferring to National Health and Medical Research Council (2019), Quality
improvements activity is a particular activity which is done to monitor, assess and improve
the health care quality in a health care organization. To improve the quality of care different
activities are implied such as, speech language pathology and clinically care related quality
improvement initiatives. Patient centred care is the care provided to the patient by the health
care professional in order to get effective result based on the health condition, physical and
emotional need of the patient. If the care is not done properly it can lead to negative impacts
on the patient’s health. As a registered nurse, it is assigned to me by the nurse unit manger to
implement a quality improvement activity. I observed that at ABC health service, patient
centred care is not provided to every patient, which results in patient health deterioration.
Hence, the quality improvement activity identified in the assignment is patient centred care
improvement, as it is one of the most important aspect that should be followed in the health
care organizations. It is observed that patient suffering from different disease needs different
care with respect to their diet, medication, physical activities and hygiene, instead it is not
followed properly in hospitals which lead to decline in patient’s health. Patient centred care
should be followed in every health care organization as it helps in providing effective result
and lead to exponential increase in patient’s health. Patient centred care also helps to
maintain patient’s safety as it assists patient to overcome their anxiety and fear through which
is done by direct communication with the patient. In addition, it also improves the mental and
physical quality of life for individual suffering from acute illness and speeds the recovery
process.

My role in the team is to supervise, that the changes required for the activity is implementing
or not. At first it is required to gather different members who will be involved in team and
after that, different roles are assigned to the team by me. To improve the quality of patient’s
centred care, different model is used which must be followed by every team member.
According to that, at first resources (patient’s medical past history) were collected through
interview. Then all the documents are collected and based on that his/her diet chart was
prepared. After that based on the data obtained, medication of the patient is revised. In
addition, the mobility issue of the patient is also taken in consideration before implementing
the patient centred care activity. After implementation of the activity, outcomes of the
activity is evaluated each day by the healthcare staff and reported to me. In case of the patient
suffering from mental disability, physical therapy is introduced.
Identification of challenges experienced in leading a team
To achieve a goal along with help of a team, it is necessary to identify challenges and
strategies are provided to avoid those challenges. In leading a team, different challenges were
encountered. During my experience of a nursing leader, certain conflicts was encountered
among the team member due to difference in their contrasting view. Everyone has different
view regarding their implementation of the activity and hence implementing the activity by
taking everyone’s view into consideration is a challenge. Management of time is also a
challenge which is observed frequently. In case of patient suffering from mobility
complication, a 24 hours nurse is required, but everyone is not comfortable to work behind
their shift timing. Another challenge observed during my experience is miscommunication
among the team members. Due to miscommunication, some of the team members remain
uninformed about the important facts regarding the implementation of the activity. To
introduce a patient centred care, interpersonal communication with the patient is extremely
support, hence a team is selected for the implementation of the patient’s centred care activity.
