Objectives
There is objective to understand the patient adherence which is an effective extent of
individual behaviour which includes behaviour regarding taking medication, executing lifestyle,
following diet and some recommendation which are effective to comply with better health which
are delivered through the healthcare service providers. It has the objective of sense of autonomy,
satisfaction along with understanding about the administering medication which should be
measured for the patient satisfaction through survey. For this, there is need for the
commencement with the provided timeframe. It also includes the objective to provide evidence-
based rationale for the self-administration medication program.
Outputs
There are several outputs which includes the policy documents, staff education package,
patient education package and evaluation framework.
Here, Policy documents which includes summary of literature review. This also includes the
patient inclusion and exclusion criteria along with patient risk assessment form. It also includes
the clear documentation processes of self-administration for patients for every stakeholder.
There is also output about the Staff Education Package that have different information about
the in-service lesion plans. It also includes the printable documents for key staff which are
associated with the stakeholders for educating patient through sharing of information.
It also includes the patient education package that may incudes printable brochure which
have different information which also includes the instruction about the self-administration along
with contact detail which can help to contact when patient may face any health related issues.
It also includes the evaluation framework which have information survey regarding the pre
and post project satisfaction for patient along with staff. There is also evaluation report which
includes the use of Kirkpatrick model that associated with organisational safety data. This data
includes the information about the medication error of pre and post project.
Inclusions and Exclusions
There is also discussion about the inclusion and exclusion criteria. Here, incusion criteria
includes the older age people between 18 and 65 years. It also includes the chronic condition
which are diagnosed at least one year ago. Here, patient should have admitted to short stay unit
(SSU). Case includes the oral medication only. Here, SAM at home without any type of

assistance. Patient is not with any type of cognitive impairment according to Standardised Mini
Mental State Examination (SMMSE) which have score between 25-30. It should be approved by
doctor or pharmacist after medication review.
Here, exclusion criteria include the injectable and schedule 8/4D medication, paediatric
patients. It also excludes the cognitive impairment and history of depression with poor literacy.
Exclusion also includes the mental health disorder diagnosis. This also excludes the history of
current delirium where patient excludes the diagnosis of dementia. It also excludes the learning
disability, symptoms related with acute renal failure, high risk of fall and list of self-
administration medication within hospital.
