In Ontario, the new vision for healthcare is well aligned with ‘Quadruple Aim,’ the globally
identified structure that makes and supplies the proper healthcare system (1). There are certain
goals or objectives of Quadruple aim. The four objectives or goals of Quadruple aim are
discussed as follows -
1. Improvement in the experience of patients as well as care provider – to change status
of healthcare providers from health worker to primary care provider
2. Improvement in the health of population – to change the need of patient from chronic
health need to unique health need
3. Reduction in the cost of healthcare per capita – to make the long-term planning
4. Improvement in the working life of care providers – to reduce stress and save energy
The Ontario Healthcare Team is substantial part to create integrated healthcare system in
Ontario. While team of the health professional works jointly to serve similar group of
individuals, then the patients would get synchronised and united healthcare. Additionally, these
objectives can be calculated by assessing the sustainable changes in above discussed goals as
well as objectives of Ontario Health Team. It can see that the there are certain key performance
indicators such as quality indicators, primary care evaluation, performance standards, conceptual
framework as well as designing as well as performance . These key performance indicators
assess how the primary care system is executing in against of the objectives as well as goals of
team at lower cost and on time (2).
