AI Summary
This health assessment report presents an analysis of two case studies. The first case involves a 76-year-old man with hearing deficits, osteoporosis, and constipation, detailing the necessary assessments such as audiometry, bone density scans, and colonoscopy, along with relevant treatments. The second case focuses on a 48-year-old female diagnosed with high blood pressure, exploring the causes, diagnostic approaches, and treatment options, including lifestyle modifications and medications like thiazide diuretics and calcium channel blockers. The report emphasizes the importance of considering patient backgrounds and tailoring treatment plans accordingly, referencing key medical literature to support the assessments and treatment recommendations. The report provides a comprehensive overview of patient care and the application of medical knowledge.

1HEALTH ASSESSMENT
The health assessment is a care plan identifies the specific needs of a person and also identifies
about how those needs will be addressed by the system of the healthcare or the nursing facilities.
The assignment is dealing with the health assessments of the patients of two case studies
Case study 1
The patient of this case study is an old man of 76 years of age who used to reside in an aged care
facility. According to his medical checkup he was suffering from hearing deficits, osteoporosis
and constipation. The nurse should do the pure- tone threshold audiometry to assess the hearing
deficits. The hearing sensitivity is measured in each of the ear for pure-tone signals. These
signals are single frequency tones which are generated electronically and are transduced through
the ear phone or bone conduction vibrator (Ohlenforst et al., 2017). The patient had been
suffering from osteoporosis so bone density scan should be done to diagnose osteoporosis.
Osteoporosis is an age specific disease and people above 35 years of age mostly suffer from this
disease. The nurses could prescribe medicines like bisphosphonates, calcitonin, hormone
therapy, and rank ligand inhibitor, analog of parathyroid hormone and estrogen receptors
modulators. The third assessment that should be done are the MRI of the spine which are done
for detecting the underlying disease of the vertebral fractures like cancer, and for assessing
whether the fractures are new or old (Effendy, Ibrahim, Mohamed & Shuid, 2015). For assessing
constipation, it is necessary to examine the rectum and the entire colon (colonoscopy). In this
procedure, the entire colon is examined with a flexible camera equipped tube (Lacy et al., 2015).
Case study 2
The patient of this case study was a 48 years old female who was recently diagnosed with high
blood pressure. The patient is a single mother of two teenagers. The main cause of high blood

2HEALTH ASSESSMENT
pressure is hypertension. The main reason of hypertension in this case is that she was a single
mother and has to take care of her children alone. So the nurse had to proceed with the
assessment by considering the background of the patient. Medicines like thiazide diuretics and
angiotensin- converting enzyme inhibitors can be applied to treat the constipation. Calcium
channel blockers help in relaxing the muscles the muscle of the blood vessels (Leung et al.,
2016). Some other medicines which are used to reduce high blood pressure are alpha blockers,
beta blockers, aldosterone antagonists, rennin inhibitors, vasodilators and central acting agents.
Doctors sometimes prescribe combination of low medications rather than large doses of one
single drug. Apart from medicines it is very important to change the life style to control the high
blood pressure. The patient need to eat a healthy diet with less salt as salt assist in increasing the
blood pressure (Parati, Ochoa, Lombardi & Bilo, 2015). The patient of this case study is of onky48
years of age, so she might get involved in regular physical activity. The patient should maintain a
healthy weight, as increased weight tends to increase the blood pressure. The nurses should also
check that the patient had not started drinking alcohol.
