Case management team, their roles, and expertise to contribute to the meeting
The prospective team involves a medical oncologist, a surgical oncologist, a radiation oncologist,
a pathologist, nurses and a radiologist. Nurse coordinators are tasked to bring patient Julia’s
concerns and information into the meeting because of the importance to of communication with
her primary health care provider. The team requires good leadership to encourage open
discussions and inclusiveness (Moorhead. et al, 2013). Team dynamics will allow trust and
mutual respect through valuing of different opinions. Administration support is essential for the
organizational process. Involvement of Julia as the patient is key by allowing her to get informed
of discussions although the presence of the representative at the meeting can be limited to avoid
biased decisions (The American Nurses Association, 2001).
The role of a care team meeting for Julia is to lay a way forward for the provision of adequate
services to the patient and her family. Care roles involve working straight with the Julia and her
family, working at the organizational level to strategize, develop and manage the provision of
services at the local setting and to work with other social and health care professionals to
coordinate and network services (Lindridge, 2017). The team aims to collaboratively develop an
individual care plan for Julia by considering all the options. The role of the team is also aimed to
enhance coordination of care and communication because cancer care is complex. The team will
source a high-quality diagnosis and treatment plan provide for a better adherence to guidelines
that are evidence-based. Satisfaction of Julia as the patient is meant to be excellent. Firstly, the
team is specialized to patients with health cancer. The appropriate member of the team has to
communicate the recommendations of the meeting with the patient afterward conclusion of the
meeting (Pieters, 2017).
