INTRODUCTION
ASE-C-POP
It is necessary to give patient required therapy in order to improve quality of life. The
drug therapy is given to overcome patient health problem. For that it is required to find out health
problem or issues of patient so that accordingly drug therapy is given. ASE-C-POP is known as
pharmaceutical care given to patient. It provides a framework of providing drug therapy to
patient. Here, main purpose is to understand need to drug related problems so that specific drug
is given to patient. Similarly, in present report an ASE-C-POP will be developed for a patient. It
provides a framework for giving pharmaceutical treatment to patient. So, this is as follows
Appropriate It has been identified from case study that there
is indication for each medication which is
provided to Mark. Here, metformin is given to
control diabetes, atenolol is given in to reduce
risk of stroke. The glyceryl trinitrate patch is
given to widen blood vessels. Similarly, there
are many other medicines given to mark.
However, in order to reduce allergy, penicillin
hypersensitivity is given. Thus, there is clear
indication of medication. Apart from it,
atorvastatin tablet is given for low cholesterol
level in patient (Hagiwara and et.al., 2016).
The glyceryl trinitrate spray is taken when
symptom of angina is observed. However,
clindamycin cap, esomeprazole is given to
reduce infection in mark. And at last sildenafil
is given to improve sexual ability.
Besides, that it has been stated that nurse did
comply with best practice therapy. In a
systematic way they have analyse mark

situation. Also, it is said for each health issue
there are different medication given to mark.
Yes , patient need a therapy of speech
pathology in order to treat swallowing. It will
be useful in treating this issue. so, mark needs
to be treated to swallow medicine.
Here, it is analysed that mark is receiving right
drug. As he is suffering from diabetes, and
other disease so drug given is appropriate.
Moreover, there is contradiction to therapy. It
is found that mark is given drugs into yogurt
and other food. So it is not proper procedure to
giving drugs. However eating of more yogurt
by mark will risk is increasing fat. There is also
increase in gastrointestinal discomfort. Along
giving aspirin to mark led to rise in kidney
problem (Jain and Correll, 2018) As yogurt
contains sugar so it can result in blood sugar
level of him. The diabetes can become major
issue for mark. As it is already not controlled
in effective way.
Safe From case study it is identified that mark is
suffering from type 2 diabetes. Hence, he is
given metformin capsule. Here, it is said that
aspirin is given for 5 years so this dose is too
high. It can led to risk of bleeding in stomach.
Mark is continuously taking this medicine. The

dose is high as well.
Besides that, it is found that there are certain
interaction in it. This is food drug interaction.
Mark does not swallow medicine. He takes it
with yogurt. Thus, taking of aspirin with
yogurt is harmful as it can damage intestine
layer.
It is found that patient is having adverse affect
of taking high dose of aspirin. He is suffering
from problem of stomach infection. The
intestine layer has been damaged in it (Pillay
and et.al., 2016). Also, there is problem of
gastrointestinal discomfort. These are signs of
taking high dose in it.
Effective It has been stated that there is no change in
mark health as he is showing various
symptoms. For example- there is problem of
stomach infection. Besides that, mark is
suffering from diabetic foot infection as well.
the metformin taken by mark has not able to
control his diabetes in past years. Thus, no
specific outcomes has been attained. In
addition to it, mark is not showing increasing
symptoms of erectile dysfunction. Therefore,
expected outcomes are not achieved. So, mark
health is not improving but rather there are
many other problems occurring in mark.
However, stable angina is being cured and
outcomes are attained (Rochester, 2019).

Moreover, it has been identified that dose and
length of treatment given is adequate. Mark is
given various types of medicines to control
diabetes, BP, also, he is taking medicine for
long time. There are no such medication issues
faced by mark. But it is found that in
swallowing of food mark is not given any
medication. Likewise, for stomach infection
dose and length of medication is short. Hence,
its length needs to be increased in it. This will
enable in proper treating of infection.
Comprehensive review By analysing the case study, it has been
observed that there are no such issues of taking
of drugs by mark. This means that he is taking
all drugs which is prescribed to him in
effective way.
But here, main issue is mark does not like
swallowing of drug. He always takes drugs
with food especially yogurt. This is problem
which needs to be solved. Due to it, there is
disease which is not being treated properly. It
is stated that mark diabetes is not in control
since past year (Rosti, 2017). In addition, the
diabetic food infection is not cured as well.
here, another issue which is identified that
lifestyle changes is not being properly kept by
mark. Hence, it is leading to rise in many
health complications.

Along with that, it is found that there is no
alternative therapy given to mark regarding
diabetes. In addition, for treating of
swallowing of drug no pathology therapy is
given. So, this has led to not treating of that
health issue which is concern for mark.
Problem list It has been identified that there are certain
problem which is being faced by mark. Here,
first is that there no alterative therapy given for
treating diabetes (Schmidlen, and et.al., 2019).
So, it has resulted in increase in symptoms of
diabetes. This is impacting on patient health in
negative way. Mark is suffering from various
other health problems as well such as
gastrointestinal discomfort, stomach infection,
Mark is not given pathology therapy as well
swallow medicine. Due to it he is takes with
yogurt. So, it might also result in rise in sugar
level and affecting lifestyle of mark.
There is another problem which is more
significant than others. It is found that mark is
suffering from erectile dysfunction. This is
completely different from other health issue.
Thus, here first priority is to solve mark heath
problem. This will be done by developing diet
plan. Then, focus will be on implementing
alternative approaches or therapy of treatment

to mark. Through that it will make things easy
Options In this for treatment of diabetes alternative
approach will be taken. In that new therapy can
be included in it. Here, risk is that new therapy
may led to adverse consequence. However,
benefit is that mark may get diagnose with it.
In this cost is considered for individual only.
This is because for mark only therapy is given
(Hagiwara and et.al., 2016).
Also, there are opportunity to integrate with
other health professional as well. in that doctor
and dietician advise can be taken. It will enable
in preparing plan for mark. Moreover,
psychologist help can be taken in it. With that
it will enable in improving health of him and
attaining outcomes.
So, in this mark priority is considered as health
professional need to treat diabetes by making
changes in lifestyle. Besides that, mark
stomach infection symptom is considered to be
treated in it.
Plan Yes, in health plan there is role of dietician and
psychologist in it. They will prepare a diet plan
and on basis of that change lifestyle.
Alongside, a proper health care plan will be
developed in it. Besides psychologist are

specialist is treating of disease so they will talk
about therapy to be given. In addition,
alternative treatment will be given to mark
(Jain and Correll, 2018).
Here outcomes will be monitored by taking
feedback from mark. Also, on weekly basis
mark health condition will be checked. Then,
the outcomes will be measured with set goal.
There is some consideration to enhance patient
ability regarding managing multiple
medication. Mark will be given a detailed diet
plan and medication chart that what he has to
take, when. The dose and length of medication
will be prescribed as well in it. With that it will
be easy for mark to manage and maintain
multiple medication. Besides for negative
impact patient it will be considered whether
therapy is as per patient need or not. The
outcome will be analysed and then decision is
taken.
