INTRODUCTION
Person centered care is a way of thinking and doing things for people by providing the
social and health care services as equal partners in planning, developing and monitoring care to
make sure it meets their needs and wants. In other words, putting people and their families on the
position to make decisions on health services. Person centered is not just given services to people
it is about fulfilling and respect their values, desires, situation and understand their social
circumstances. Present study will be base on the reflective approaches in implementing person
centered practices. Therefore, the report will further outline the different concepts and theories.
Also, this covers the different more topic like comparing social and medical models for person
centered in health practices. Also it defines the different concepts and making the study more
productive towards the aim of the study. Study also emphasizes the current policies and

As according to the act 2008 health and social care and regulations 2014. The main
intention of this regulation process is to ensure that people using a service have care or treatment

should get the best quality of services and make sure the things more effectively done in efficient
manner as per the need or demand of the patient health. However, it helps to ensure the best

regulation providers must make sure that, weather this position has discovered the effective
working out goals to ensure the best services to the patient. Also, provider has to make sure that

Under the Regulation 9 the main intention of the care taker is to give personalised
treatment to the patient as per the condition and according to the requirement (Norwic and Eaton,

and practices. Such challenges that faced by the care professional are cost issue which is the

Scenario 1: Give a brief description of the patient or services user or individual in need
that you have provided person center to:
Being a Domiciliary worker, I have recently deal with two cases. One of the cases is on
Alzheimer's patient. He is person of 60 years old live in Australia alone. His wife left him at the
age of 50 from last 10 years he is all alone. Due to frustration and anxiety he become angry
man. All such situations or conditions affect his mental health. When I met him, he is a person
who suffering from Alzheimer’s. I identified the situation and evaluated that the reason behind
the mental health issue is because of loneliness. Being as a care giver, I have done several
things and use different intervention of person centric that helps me in the process throughout.
Along with that, I have used technique or Person centric approach which is VIPS which stands
for Valued, Individuals, Perspective and Social. All such elements together form a good
support for the person who suffers from Dementia. Valued define that patient should get valued
from his care taker while giving services. As I do throughout my session. Reason, Alzheimer's
patient is very sensitive and emotional. He requires special attention from their care giver. So I
always makes sure that John will never get alone and think negative thoughts. Thirds is
perspective of the person with dementia must inform our understanding which is also very
much necessary for patient as well as for mine. Last social aspect of this intervention wants to
describe the positive surroundings and relationship with patient. VIPS is the approach of
Person-centred care which helps me to treat John.
Another approach that I have been used during the process is Palliative Care which is another
significant for John to give quality care. This intervention mainly focusses on three aspect first
is life is affirmed, Distressing symptoms of whatever sort should be actively treated whilst
maintaining quality of life and Holistic care. In this intervention I used to handle John with
holistic approach in which I have prepared sessions with Johns friends, neighbours which
further give happiness to John. Holistic approach is the intervention which gives social and
emotional support to the patient.

Key Principles of Person-Centered care
Affording people dignity, compassion and respect: This principle of person-centred care
support me to maintained the good relationship with John. I never performed any rude
behaviour with John which make him nervous and depress I always listen and understand their
feelings.
Offering Coordinated care: while during the health care session I used to help him in the
activity in which they have performed. I will support him mentally and physically in throughout
the process.
Offering Personalised care and support: In offering personalised care so I used to make
health plan and routine plan for John during the whole sessions.

As with my second case with a lady of 55 years old. She is suffering from diabetes and live with
their family in London. Despite that, when I met her she I analysed that she is very energetic
and influencing towards making things a kind of energetic and innovative person. I spend a
good time with her during her treatment. I really enjoyed and facing less issues by treat her in
the duration of time. I have been using different intervention style throughout the process.
Which helps me out in the duration of time. After that I have been used different interventions
for Marry to make her more stable and help her in the best possible manner and growth. I have
used model or approach The Chronic Care Model this is the model which helps to use with the
patient of chronic disease. I have been used different approaches and task for this goals in order
to meet my own aims and purposes with the case. The basic and main purpose of this
framework is to enhancing care delivery by identifying essential components of the health care
system that can be modified to support high quality, patient centred chronic disease
management. Along with that, it also makes the things more clearly mentioned and make
different in the situation. Besides, this model helps me out in different context. Which certainly
based on the issues. While implementing this issue and system I will make sure the following
activities. Also I visibly support improvement at all level of the organisation. I will make her
systematic and make other person. Mobilize community resources to meet needs of patients.

better way. I always give her respect and support in their difficult process which makes things

different aspect. On the other sides, it is less effective and low generating the work out goals in
different form. During the whole process I help her out from the situations and made different
following results. Also I support her in their daily struggles to take medicines.
Offering Personalised care and support : Being her care taker I have give her several
personalised care that helps her to adjust and stay happy with the issues. In other words, I
provide her to decide what she should eat and what should not. Also provide guidance and
suggestions on their daily routine activities and also make her convenience for exercises.
Reflective statement on both scenario
From the above two scenario I have learn so many things and get a good experience from the
two cases. I have gained knowledge about the principles and practices which should need to
understand and make the things more appropriate. In other words, I explore the assessment of
pain as conducted with one 60 years old man and one 55 years old lady. I tried to give them
support in their respective issues. I implement Person centered principles and also applied.
Along with that, it also helps to give matter and formation with effective duration of time with
the help of different intervention process. In order to reflect this statement I have been using the
Gibbs model that helped me out to present my feelings and study that I have conducted with
two people. It helped me to promote what extent I as a nurse should recommend analysis. I had
visited with two different people with different issues. I found how people and person have
gained the different aspect to performs and manage. I have used provisions and my work
principles that helps me to promote in my career development task and also helps to understand
the different business aspect of the study in different manner.
