INTRODUCTION
In today's time every individual wants for growth and development of personal and
professional skills. It can be achieve through set the goals, strategies and desired outcomes of
learning and training (Lumpe, Czerniak and Beltyukova, 2012). To enhance development of
personal life, every individual require to self-assessment and identified key area to improve skills
for themselves. In this report covers different things through every person can develop personal
and professional skills. Furthermore, it assesses current skills' ability and learning style based on
SWOT analysis. In addition to this, it explains the nature of different professional relationship in
health and social care context. Beside this, report analysis own role in minimizing barriers to
effective teamwork in health and social care practices. At last, it covers evaluation of the
effectiveness of personal development plan as health care practitioner.

the mental health patients; I ensure that I am treating them positively and I am behaving calmly

I keep my personal values and culture separate so that I can focus on professional contexts. My
previous experience aids me to avoid all such things that can generate issues among patients and

1.3 How new development and changes in personal values impact HSC working
I always prefer to adopt changes and new things into account so that to enhance own
learning level. This helps me to treat all the patients effectively and at the same time it is also
beneficial for enhancing the opportunities of team work and success. I am also adopting new
tools and techniques as per my job roles which assists me to deal with technological facets;
hence I can say that is significant for me in numerous ways. Similarly, in work context,
technological dimensions reduces challenging and complex issues. Henceforth, as a result I am
able to adopt new things successfully. At the same time, this also aids me to contribute more in
goal achievement aspects. However, changes in personal values sometimes creates differences
among service users and providers and I have seen such issues when I had to deal with such
patients.
Values are work are essential for me because that directs me to perform all my roles and
duties aligned with the organizational aspects. At the same time, values at work is also crucial
because that aids the individual to consider appropriate environment at the workplace and
similarly this can also strengthen the health care services. Nonetheless, I always follow the

policy of work prioritization in which I segregate all my roles and responsibilities and according
to that, I perform all my actions. While having any of the urgent cases, I delegate responsibilities
to all my team members so that together we can manage each and every aspect in prominent
manner. Moreover, this also changes my values and opinions which I follow at the time of
conducting diverse roles and responsibilities.

The learning style of Peter Honey and Alan Mumford includes four major stages which
are mentioned as follows:

The subsequent model assumes that people have preferred learning styles but the styles
are usually based on context and experience. They are not locked into any one style; thus as per
the situations learning styles could be changed.
Activist learns from personal experience, they also conduct practical games and activities
and they emphasizes on energetic aspects so as to learn more. They generally avoid to work
alone.
Further, Reflectors take time to think and observe things around them which helps them
to conduct their actual roles (Takemura, Kielmann and Blaauw, 2016).
Theorist accentuates majorly on theoretical concepts for the purpose of undertaking
specific decision and the last style that is Pragmatism focuses too much on upcoming stages.
This also prepares the learners for future uncertainties.
On the basis of Honey and Mumford’s learning style, I would prefer to learn through
theorist because I consider it the suitable learning style for me. Moreover, this will also enhance
the knowledge level and thus I will be able to get more career building opportunities in health
care setting. However, I also need to focus on other studies so that I can learn practical things
related to my job profile.

3.1 Nature of different professional relationships in HSC
In health and social care there are various professional workers who serving health care
services in effective way (Harris, Graham and Adkins, 2015). Different types of professional
working together because their serving method can be differ for reduce illness. In this context,
professionals working as General Practitioner (GP), nurse, Community Psychiatric Nurse (CPN),
health visitor and occupational therapists, etc. Along with this, a social worker is a person who
provide information and advice about the services which are available for users. Other
professionals is a multi-disciplinary team who are working with the information which is given
by social workers.
Team is working for solve users’ problem and give them benefits which they want. Thus,
there are effective communication require which play important role for development of different
professionals. Along with this, other staff members are support to workers to organise plan to
take care of users. As results, all members are working in chain which helps to achieve goals of
the firm (Boninska, Borisova and Gagova, 2015). Beside this, counselling skills helps services
users who are suffering from depression, bereavement which lead to the social worker making
referrals for service users.
Professionals are working to encourage users through working together. In this context,
they are works as team for providing quality services to services users. Along with this, they can

health education to patients. Their main role to deliver effective services to protect and
care for services users. Their responsibility is to meet the emergency needs and
requirements of the patients. They are accountable to work as per the defined guidelines;
hence they have to meet the health changing needs of clients.
Nurse: Nurse plays a significant role in hospital, clinics and private practices. They are
working in the biggest health care occupations. They are play role includes
communicating between patients and doctors, caring for patients and supervising health
of service users. While working with different team members, it is crucial for the nurses
to coordinate properly with others so that prominent services could be delivered
accordingly. They have to show positive responses to each and every need of the client.
Community Psychiatric nurse: Their main task is to administering medication and
monitoring patient progress in their daily life. They are pursued course to deliver
effective services on the basis of regular surveys. With the setting up district general
hospital, CPN work effectively in HSC.
Professional care: Profession care of HSC work to provide care and medical services to
patients for their daily life. Professional care are concern with responsibilities for deliver
services according to patient quality. They are accountable to consider the regulation
made for medical health services; therefore in this respect professional care standards are
mandatory to follow.

3.2 Evaluate personal effectiveness in promoting and supporting the rights of individual
I am aware about my personal values and beliefs; thus I know dignity and rights of all the
individuals. In order to support and promote right of individuals, in will have healthy interaction
so that is get to know the issues that are faced by them. Accordingly, steps will be taken with the
help of which proper support can be provided to individuals. For example, I will have personal
interaction with individuals and try to identify the problems that are faced by them, then that
information will be delivered to management so that they will be able to take proper steps.
Further, interaction will also enable me to work in partnership with people who make use of the
services. I work as a team at Care UK which helps me to support all my team members and
which also assists in reaching towards the team objectives. I maintain professional relationships
with my colleagues which shows that I respect and treat people effectively. Since, I work as a
team; so I am accountable to maintain friendly environment at the health care setting. For
example, when healthy environment is delivered, then this motivates other to perform. I learn
things from others which not only enhances my skills; but also assists me to develop team bond
with people (Styhre, Roth and Roth, 2016).
While working with different partners, I had maintained proper coordination level so that
we can share team aspects in prominent manner. Through this, we were able to meet the team
aim and objectives. However, I also experienced problems due to difference in opinion from the
team members; but then discussing each and every aspect, we came up with suitable resolutions.
I always ensure that all my team participants are following professional code of conduct.
I work as a counsellor as well because most of the time I have to deal with mental health and
depressed patients. Apart from this, my job profile also directs me to meet the needs and
demands of patients since as a health care practitioner I am liable to do so. While undertaking
team decision, I invite all the team players so that effective decision for the team can be taken.
This not only enhances team bonding; but also reduces the opportunities of conflicts and
grievances (Strasser, Kam and Regalado, 2016).
3.3 Ways to resolve issues in professional relationships
