INTRODUCTION
The pharmaceutical care plan is defined as a written individual and comprehensive basis
of medication therapy which is usually planned to provide the relief from the disease is with the
identified goal which is clearly referred as the therapeutic objective. As per this, the
pharmaceutical care plan is individually analysis and evaluated for the patient who is facing the
issues with any disease for the longer period. In this, the pharmaceutical care plan which is
available to all the pharmacist by caring for a patient is well updated and take a major step by the
overall analysis of patients who is facing the issues as concerned with their health agenda
(Khaund and et. al., 2018). As it is very important to understand the evaluation of physician and
make a strategic plan which ensure the common goals which is compilation of patient and in this
the patient must focus on their health concerns and complications and they used to take some of
the diabolical structure in order to share their views and thoughts. The recent complication which
is faced by patient. They must share with the physician in order to make a strategy and action
plan which is helpful to eradicate the issues. In this, report infection because as per current time
it is well analysed that there is various disease which is associated with the infection. The
infection has many various causes but their source is negotiable and sometime they are
identified. In this, there are proper medications are used and interventions is also approach to
retaliate the issue. So, as per this, the pharmaceutical care plan is well identified and evaluated
through this section.
MAIN BODY
Reason for patient selection and aim in managing the patient
This helps to provide the big knowledge about the patient selection in which the main to
manage the patient who is facing the issues with the infections which may be major or minor. In
this, the optimal medical management is needed for the patient and make a clear understanding
of the medicine which is prescribed. As per during the infectious order in this the medicines are
well analyzed and monitor their side effect and adverse effect which can cause additional
systematic revolution for the patient. In this, the patient must answer the questions of physician
in order to take proper management and medication (Kuhar and et. al., 2018).

of 40 years and he is a non-smoker in nature and their body mass index shows up to 39.6 which
is overweight in nature. The metabolism process of the patient is low and we consume alcohol a
lot. As per this, the use to take the diet which is oily and low fat with low cholesterol factors in
this. The impression is well analyses that the patient is struggling from allergy and also so he is
dealing with the infection of the lungs and various areas which create rashes and skin infection
(Leung and et. al., 2020).

Pharmaceutical issue plan and propose outcome
The pharmaceutical issue plan is used to elaborate the action plan and their outcome in
order to create the intervention which is necessary for the patient was facing the issues with their
health concern with the infection. As per this, there are various diseases which is above
mentioned in the issue care plan which is well optimised to control the infection which is period
or replicate in the host cell after taking the thermostat is which is create the analogue and
complicate situations for the patient in order to raise the pharmaceutical issue plan. The actions
are needed in order to take by the medication such as taking the example of Lopinavir which is
act as a anti infection for the various disease which is associated with infection. They include
chickenpox, cough, diphtheria, HIV, influenza and so on. These are the disease which is usually
caused due to the infection by poor sanitation or due to the mutation which is first inside the
host. In this, usually the infectious are the causative agent enter in the body of host and maintain
the environmental structure and start to replicate itself and make the body disease full. As per
this, to control this there are various medication which is ever mentioned such as acyclovir,
erythromycin, ampicillin, penicillin and so on. These drugs are antibiotic in nature and usually
racist and prevent infection to replicate and spread inside the body (Rebmann and et. al., 2021).
Patient management plan
The patient management plan is usually work under the influence of proper medication and
self-management. As per this, the exercise and diet plan are also well helpful to prevent or treat
cure the disease which is caused due to the infection. In this, the interaction is replicate in the
body and multiplicative their foreign cells in order to make our environment which is appropriate
for the virus or parasites. In this, the individual needs to care of that and used to take proper diet
and food which helps to minimise the risk factor which is regenerated by the disease or the virus
which is inside the body in order to this. It is well analysed that there is various condition which
make the patient more complicated than the diet plan is not appropriate which is featured for the
body to control the replication (Volgenant and et. al., 2021). This condition creates various
consequences which is faced by patient and the management plan which is associated that
include yoga, proper diet, self-management therapies and so on (Wilson and et. al., 2020).

CONCLUSION
As per the above discussion, this is well analysed that the condition of infection can cause
various disease which is create light fitting issues for the individual or the patient. In order to
this, to remove the consequences or eradicate the issues of infection we need a proper care plan
in such a way which help to retaliate the condition which is complicated the complex situation is
arises when the pharmaceutical care plan is not proper in nature due to the combination of drug
or for the medication. As per this, the proper regiment and the combination of drug is used to
control the spread of infection.
