 Advanced care plan refers to the process of planning which can be helpful in the
future.
 This is the one that can be highly effective and have better response towards
providing better healthcare services to patient when they are seriously ill or
injured and are not able to communicate their preferences along with decisions.
 It is essential to take care of individual patient health which may include to
choose substitute decision making according to patient decision making
preferences and their advanced care (Ufere et. al., (2019)).

Effect of ACP on patient and their family
 There is high impact of advanced care plan for palliative
patient and their family. This is the one in which
healthcare professional make an effective care plan for
the patient specialised for their health improvement.
 Through getting ACP care, it can be helpful in improving
patient health and associated with delivery of effective
care that can provide satisfaction to patient and their
family regarding delivered care services (Kuusisto et. al.,
(2020)).

Ethical decision making in ACP
 While delivering patient healthcare services, it is essential to take
care of quality health services which may include taking care of
decision making which should not violate individual rights.
 Advanced care plan can be effective in taking care of patient
adequate communication, their autonomy, issues along with
symptoms which are associated decision making (Bradshaw et. al.,

 Nursing staff should take care of patient medication and should make

 Communication refers to a therapeutic tool in advanced care

with security for patient and their family which are effective
for patient health improvement.

enable to connect with patient which can be helpful in
