INTRODUCTION
The health assessment of population refers to the measurement, analyses and monitoring the
data of health of population. It also provides the determination of knowledge about status of
health population (Hatef, 2018). In this, the health inequalities and social determinants which are
present in health are also included in this. In this report, the health assessment in Brent and
Harlesden is covered and there did demographic status and determinants of health along with
depreciation are also covered in this. The key sources such as status mental health services of
community health and wellbeing are also covered in this report.
BACKGROUND
The community health needs assessment refers to a process that occurs within community so

developing various components version community such as planning organizing developing the
goal prioritizing health issues, etc. The monitoring is defined as the process in which data about
health is collected with the help of dynamic process. This is occur for a continuous period from
which the determinants of health can be analyzed that is occur over population in a time period.
The surveillance is defined as the process from which data can be recorded in a more
intensive manner then monitoring. From this the disease and health issues that are coming within

data from which health status of community can be derived. It also includes in the collecting data
from which health issue of community can be priorities (Maylahn, 2019). Along with this, the
communities also plan various further improvements from which we can evaluate their progress
of health assessment needs. From the developed in progress the health status of community can
be enhanced by considering factors that are needed for health of communities.
COMMUNITY HEALTH PROFILE
There are two main health profile are occur with the public health of England which are
Brent and Harlesden.These two are referred as short and long profiles of community health.
There are four categories from which the health of communities can be assessed. With the help
of these categories the health of population can be measured properly (Schuurman, 2018). The
demographic status of Brent occurs in which there are approx. 66.5% of population occurs. The
life expectancy within this profile is good and expectancy of life towards health is similar to the
average of national. There are use number of population who are living with cancer heart disease
and cardiovascular disease. These diseases occur within the population of blue 65 year old as a
premature death. The demographic status of this profile was that approx. 91 people are suffering
from cardiovascular diseases from 100,000 people. This profile is rank about 120 from all the
150 local authorities under are present in England. It is also named as fattest as higher number of
people or adults are suffering from overweight obesity from the level of England. The
populations of this people are approx 56.4% who are mostly adults. The population that are

of need can be assessed by individual level. The expressed needs are referred to the term of
identification of services that are used by population. From this the information about a person
by observing them with their future services can be done. This is also influenced by the various
services availability. The felt need help in recognizing the elements of need in terms of
individuals according to their necessity of needs. This needs help in identifying at local and
national level by having care representatives. The theories of population trends and traditions
including theory of health needs provides help in interrupt the community profile appropriately.
As these needs also help in identifying and assessment of meat that are needed within a
community. We can effectively evaluate the needs that are beneficial for people of communities
or not.
The community assets are defined as the resources that are collectively present in which

CONCLUSION
From the above report it has been concluded that there are various limitations occur of this
profile as they does not have proper resources from which they can supply resources to their
people of communities. These two profiles do not have proper employment and are emissions
from other sources from which their limitations of providence of resources occur. This becomes
limitations in their profile from which they do not able to retain the health of population
appropriately. These two you also so have high rate of services which is also occur as limitations
among them from which they are unable to retain proper resources. The identification of each
individual needs also help in developing other information that helps in assessment of
understanding the needs of local health. With the help of this, the needs of local people can be

communities. The involvement of this helps in understanding the needs properly. With the help
of this the needs of health can be rapidly provided to individuals of communities. The use of
different needs method can also help in assessing the needs from which the health of a material
can be enhanced. This provides better understanding about the needs of health of local people.
