Post-acute care denotes to a variety of medical attention services that cares the
individual’s constant repossession from illness or managing of a chronic illness or ill health
(Alcusky, Ulbricht & Lapane, 2018). The process is done by succeeding a hospitalization for
wound or illness; many patients need constant medical care, either at house or in a dedicated
facility. The acute care services include hospitalisation for long-term cure of chronic diseases,
rehabilitation and also for psychiatric condition. The services also include ambulatory care,
agencies who provide care at home and also nursing home for care. In the case study the
patient is an older person and is discharged from the hospital and advised to be admitted in
nursing homes for long term care. Nursing homes are places which provides care for elderly
person which provides home or resident care which includes services like occupational
therapy, memory care, physical activity and helps in betterment of treatment. The United
States comprises of mostly three types of nursing homes such as intermediate care facility,
board and care homes and finally assisted living facility. The most common recommended
nursing home in New Jersey are Abigail House for Nursing and Rehabilitation, Actors fund
home and Alaris Health At Belgrove (Medicare.gov, 2020).

that provides health support to elder patient with disability without any restriction of income.
The Medicaid services are programs that helps the elder person with low economic stability
and provides monetary support. This home also helps the patients with both resident and
family counselling. The organisation provides a good quality of resident care which regularly
reports to the Centeres for Medicare and Medicaid Services (CMS) and based upon the
reviews the rating is conducted depending upon short stay and long stay in the nursing home.
The staffing members of this organisation include registered nurse, vocational nurse, physical
therapists and also nurse aides. The organisation have not faced any denied payments or any
penalties in the last 3 years (Medicare.gov, 2020).

Nursing homes provides quality of resident care, which helps the patients, especially
older ones to get cured or receive a long-term care. The patient in the case is an older adult
and is also a Medicare patient which indicates that the patient has a kind of disability with no
restriction of income. The Centers for Medicare and Medicaid Service, which is a component
of Health and Human Services of United States, provides support to this patient to provide
quality care. The service will pay the bill of the patient which is covered under Medicare Part
A skilled nursing facility welfares. The payment will be paid on the basis of period of stay in
a nursing home. The bill will be paid for less or 100days stays in the nursing home, after
staying 100days the patient comes under the category of long-term stay. The service provides
all short of care such as private rooms, physical therapies, speech therapy, medications,
counselling and social services (Medicare.gov, 2020).

belongs to the Medicare Part A skilled nursing facility welfares. The rate includes from Day 1
to 20 it is $0 for each profit period, from Day 21 to 100 it is $176 coinsurance per day of each
profit period and above 101 days it is all cost (Medicare.gov, 2020).

The patient falls under Medicare Part A skilled nursing facility welfares which
includes treatment plan of short stay with cost covering day basis. The rate includes Day 1 to
20 is $0 for profit period above 100 days it which is considered as long term care which
includes 100% payment of the services (Medicare.gov, 2020).

The ethical implication lies in the long term care of the patient and nursing home
stays. It process fails to promote resources for long term stay for the elderly people which
