Support services and resources in Aged care environment
The aged people require special attention and in the environment where only elderly
people are living, it becomes important to provide certain kind of support services to the people.
There are various services that can be provided (Brownie and Nancarrow, 2013). However, the
two main support services are as follows:
Residential services: In this support service, aged people are allowed to live in the
residential care home where minimal amount of money is charged from the family as it
run on not for profit basis. Further, all the facilities are arranged in order to make their
stay comfortable. They are provided with 24 hours support care through nursing and
helping them in conducting their task (Stewart, Georgiou and Westbrook, 2013).
Transport assistance: The person is eligible to get free or low cost transport services in
aged care environment. Aged people are not able to travel alone and attend their
appointments and social gathering. Hence, they are accompanied with the person who can
take care of the elderly person and take him to the desired place.
There are various resources that are required to be made available in the aged care
community. The main resources are as follows:
Physical resources: Making basic resources available to the elderly people which
includes, wheelchair to take them from one place to another, comfortable beds, ramps,
handrails in toilet, anti skid flooring etc.
Supporting resources: Providing guideline booklets that further help in giving them
directions about their diseases and also about different necessities available in the aged
care environment (Howe and et.al., 2012).

Primary health care and services that can be offered to Kate
There are various services that can be provided to an older person. It is important to
assess different detriments of the chronic diseases and deciding the plan according to it. It
includes, clinical services in order to treat their disease and providing a supporting hand to the
person. Furthermore, promoting prevention to the illness among aged people through better
communication campaign is another activity organized as a care (Duckett and Willcox, 2015).

Kate being a diabetic type 2 patient, there are various services and care that will be
offered to her which includes the following:
Managing the lifestyle by improving eating habits and physical activities
Constant monitoring of the blood glucose level as high glucose level can deteriorate the
health of the patient further leading to act of confusion and clumsiness.
Preparing a diet chart which will be required to be followed by Kate (Primary health
concerns for older people, 2012).
Prevention of falls as it takes quite lot f time for a diabetic patient to heal the wound
rather than the normal one.
Constant reporting the symptoms of hypoglycaemia and hyperglycaemia state in Kate.
Monitoring the medicine intake and ensure that it is continuous and appropriate.
