1. Introduction
A communication plan can be referred to as a significant policy driven approach that helps to
provide significant information. Organizational stakeholders gain this type of information and hence
proper information is required to be delivered and which communication channels are required for
delivering that specific information [4]. An effective communication plan can anticipate on the type
of information that is required to be communicated to the specified audience segments. The
following report outlines a brief discussion on the communication plan for the case study of
Improving Public Health Informatics. The County Health Department or CHD has made a strategic

communicable disease outbreaks [2]. For ensuring success they have made a communication plan

2. Discussion

3. Conclusion
Therefore, from the above discussion, it can be concluded that the communication plan of
County Health Department is perfect and it can increase the capacities for program evaluation. The
integrated public health informatics system can easily detect the infectious diseases and possible
future outbreaks can be done within a timely manner. Moreover, correct decisions can be
undertaken by involvement of this particular system. A collaborative process is being followed for
