This assignment presents a questionnaire designed to assess the impact of extracurricular activities on students' academic performance. The questionnaire includes demographic questions (gender, age, height, and weight) and a series of close-ended questions that explore students' perceptions about the benefits of extracurriculars. The questions cover topics such as confidence, physical and emotional well-being, academic performance, social skills, self-esteem, anxiety, disruptive behavior, time management, and parental/teacher encouragement. The questionnaire uses both nominal and ordinal measurement scales to gather data on student opinions and experiences. The objective is to validate the hypothesis that students participating in extracurricular activities perform better academically. References to supporting literature are included. This questionnaire will be distributed to 10 individuals to collect data for testing the hypothesis.
