Practical Assignment
AI Summary
This assignment presents a detailed recovery-focused nursing care plan for a 31-year-old woman, Rachelle, experiencing postpartum depression, anxiety, and suicidal ideations following the birth of her daughter. The care plan addresses multiple issues, including severe depression, self-care deficits, and challenges with daily activities and work. It utilizes the HONOS scale for assessment and identifies the consumer's strengths, such as self-awareness and family support, to develop interventions. These interventions include cognitive behavioral therapy, schema and dialectical behavioral therapy, art therapy, and interpersonal therapy, implemented by a multidisciplinary team. The plan outlines specific goals, interventions, responsible parties, and timeframes for each identified issue, aiming to improve the consumer's mental health, self-care abilities, and social functioning. The assignment emphasizes a recovery-oriented approach, incorporating the consumer's strengths and promoting a holistic approach to mental health care, with references to relevant research supporting the chosen interventions.
