INTRODUCTION
Formative assessment is wide range of method that tutor used to conduct in process
evaluation of the student learning need, progress regarding the subject and lesson. Self reflection
is a type of activity which depend on the thinking about the own behaviour and feelings. This
will be main reason to understand their own perspectives towards the learning. Reflection is
playing important role in the social and health care for enhancing the personal development in
this way, it also leading self awareness. Reflection is help for improving the health care of
patients (Husebø, O'Regan and Nestel, 2015). Main goals of this assessment is to monitor the
student performance and providing the feedback according to their learning capabilities. This
report will discuss about the importance of referencing in the academic writing. This project will
cover the entire meaning of academic writing that help for improve the skill and knowledge of
student. Further, it will describe thee plagiarism and also link between the referencing and
plagiarism. This assignment will discuss about the importance of reflection in health or social
care project.

Formative Assessment
Formative assessment is type test for student to practice regarding the improvement and it
is just like a meaning home work. This process will help for check the understanding of people
and teacher will guide to student for develop decision making power about the future instruction.
Formative assessment also differ from the summative assessments pattern because formative is
not focus on the graded but it depends on the knowledge (Almgren, 2017). This process can take
the form of many things and this is simple writing skill that represented by drawing, quiz and
short discussion etc.
Formative project can applied in the primary health service delivery and this is the initial
step in the government which is initiative towards the health service which is provided in
different cities and countries. Formative assessment includes diagnostic testing which requires fir
identifying the knowledge of student. They have specific range of informal and formal
assessment procedure that conducted by teacher. This assessment will cover the qualitative
feedback for providing the teach to the student according to their performance and detail of
content. There are some important formative assessment example that providing the brief
description in proper manner.

Model of Reflection-
Gibbs reflective- I used this type of model for creating an effective and dynamic documentation
it is based on the reflective cycle that encourage following systematic about the different phases
of activity and experience.
 Description- I can use this concept to describe each and every requirement in the
formative assessment. It is an essential for specific criteria that manage the entire
processing.
 Feelings- I always thinking about how to complete their task and operations in proper
manner. It depends on the feeling of person that different opinion towards the
assessment.
 Evaluation- I can easily identify that what is good or bad for me. It really helps to build
the understanding towards the assessment.
 Analysis- it is a last stage that help for me to analysing the entire situation proper manner
afterwards, I can take right decision.
 Conclusion- At last, it concluded that learning methods are useful for providing the
guidance to build their skill and knowledge.
 Action plan- I create action plan and also applied the different plan according to the
requirement. I easily learn new things for applied logics (Smith and Roberts, 2015).

For Example- In my college, I attain the placement drive and I was working as surgeon wards. I
was working under the experience supervision to provide the lot of information. Sometimes,
there is abdominal surgery and I asked to remove her wound dressing but doctor assess it on the
ward round (Cooper and Wieckowski, 2017). In this way, it can be understand to use the
feelings, Evaluation, Analysis, action plan, conclusion etc. I will develop the own plans for
managing the entire operations and task by using Gibbs reflective model.
For Example-
Analysing the work of student- A great deal with the information that student can leant from
their test, home work and quizzes etc. it helps for identifying their thinking capabilities and
abilities towards their working. They also gain the knowledge with the help of formative
assessment test.
Strategic Questioning- Questioning strategy used with the group of student, individual and
entire class. It will be more effective than formative assessment because it helps for
understanding the student skill and knowledge towards their subject. High level question requires
for identifying the depth knowledge and thinking of students (Davies and Challis, 2018).
Class room polls- Polls help for student to quickly give the response in accurately. A silent
perfect for the shy student . They also response quickly in the class rooms. This method will
useful for check the understanding of mobile technology.
Purpose of Formative Project-
The purpose of formative project is to provide the progress feedback and also useful for maintain
the program that implement in the different possible ways. Another ways are to share the lesson
and student learn by individual local people. This project will help for reporting the
accountability of the formative project. There are different types of process to maintain the
formative evaluation in proper manner.
 Capacity development and learning for the system improvement.
 Performance monitoring, communication, evaluation and reporting.
 Enhancing the project implementation.
The goals of formative assessment is useful for monitoring the student learning to provide the
ongoing feedback. The teacher will instruct to the student for improve their learning capabilities.
The student identify their weakness and strength of particular areas and it is required for

managing the entire process targeting the areas that needed for work. Faculty and teacher will

Importance of referencing in Academic writing-
Academic writing is a clear, focused, concise and structure that represented the effective
style of expression. Academic writing is conducted in different set of forms and genres that help
for investigate particular knowledge. It is mainly circulates within the academic world but the

Referencing is the important part of academic writing that provide the actual source of
authentication whereas information useful for the writing purpose. This will create relationship
between the plagiarism and referencing while both are interconnected to each other. Citation is
not simply to avoid the plagiarism in the academic report which is important role playing in the
writing. Referencing is the best way to provide credit to the writer to cover all the ideas and word
that borrowed from particular sources ( Fernández and et.al., 2017). There are some important

Personal reflection project towards the health and social care project
I have personal experience in the project of health and social care that build the
understanding towards the society and communities. Health and social care programme
conducting in our university. I also participate for providing the service to the people those who
are needed. This project was important for university that help and support to other people
regarding their health service (Kennedy and Yaldren ,2017). I have specific role in the university
to work with coordinator to create safe, rewarding ans supported to the volunteering opportunity
which is beneficial for students. I and different volunteer participate on this unique project that
enables the student to develop their skill and knowledge. In this way, they also shared the
decision marketing and could be engaged with the inter-professional learning. It also created the
opportunity for student form disciplines to come together and completing their task to share their
experience thoughts to others. I must consider the different implication of volunteering that help
for future development to integrated with the health and social care service. This project will be
beneficial for student and also providing the long terms advantages of large number of peoples. I
also support the other people to understand their need and requirement (Hignett and et.al.,
2018). Afterwards, solve their issue and problems in proper manner. I also playing important role
for managing all the facilities and services. I have involved in the teaching session to guide other
people and aware about the role and responsibilities towards the health care services. I have
completed a diploma level module that help for develop the skill and knowledge. For being
coordinator, I have many responsibilities to arranging the classes for people those who coming

form attending the seminar. I found that I was enjoying my work. It will be the great experience
for identifying the different issues that generating in the health care societies and communities.
Afterwards, end of the seminar, I was wondering that they had invited the right people for the
conference. During the break time, it also meets the colleagues and relative of different
backgrounds. They gave me confidence for the actively participate in the discussion. In found
that really effective for me to boost their confidence level in proper manner. After some time, the
discussion could be started again and also presenting the issues or problems regarding the health
care. This will helpful for other people to promote the health services all over the world.
 Mental health and psychological requirement placement and usual unit.
 Elderly care and it was able to provide the best example to their colleagues.
I also able to connect with the people to provide the information regarding the conference and
classes. Our university is maintained the facilities and services in proper manner (Hvalič-
Touzery and et.al., 2017). I also help for people to convey the messages for one place to another.
I had great experience and enjoy a lot. I also learnt many things related the health problem and
social culture. How health issues impact on the entire social communities. I have learnt the
different aspects of health service that acquired the knowledge and skill. I really enjoy the entire
conference in effective manner which also beneficial for improving the knowledge and skills.
Importance of reflection in health and social care project
I have been work as social and health care in the university and also going into the
qualification training which help for understanding the need and requirement of social
communities. Health and social care is the important sector that help for providing the best
services to the people. I started as work in university of performs different operations and tasks.
My social work experience involves working in wide variety of setting and also including the
families and children to provide the awareness regarding the health issues. Our team provide the
guidance to people to concern about their health and social culture. On the other hand, I moved
into the another community which will be related to the mental health problem. My career being
changes according to the requirement. I have leant many things and build experience towards the
health and social care project. I feet that social work is complex and they have clear process or
approach to manage the entire functionality of project. Changes have continues process
according to the project and their management system. Social and health care manager response
to guide towards their role and responsibilities (McGuinness, Maxwell and Harper, 2017). They

also identifying the need and requirement of health project. According to experience, I conclude
that project management of health and social care project are coordinated with limited resources
and have complex deliverable sector value. I used different tools and platform that can help for
project and build the confidence towards the critical, challenging and honest includes in this
project.
I faced difficulties in the health and social care project when I was trying to keep records
all the necessary informations that required for management. Sometimes, it will be created
tension between the operational style and the management system of health and social care
project. I have seen that change is constant and manager cannot involved in the project at the
certain time. I think it's a great and challenging issues in the resource management process. It
required for produce the efficient services and also balancing the quality, requirement of budget
or integrity and resourcefulness (Wei and et.al., 2017).
Importance of health and social project-
The health research has high value to the social community. It can be provide the
information regarding the diseases, trends and risk factors etc. There are important importance of
health and social care project. The project objective to formulate policy to help for socio-
economic inequalities that linked with the health behaviour.

department.
 Collaborate and securely data- According to my opinion, this is the best way to share
the data and information within hospital, production unit and laboratories etc. When I
have been work in the hospital so that I have seen different department communicate with
other staff member. It will require for providing the safety and security because

modern technology that support the management system of hospital to secure the
information regarding the patient and client (Milligan, 2017).
 Continuous access – I have access the different type of sheets and categorize according
to their need and requirement. Health and social care project is the best part of
organization that help for increasing the productivity and profitability. Sometimes, I have
seen many technical issues and problem arise which is directly impact on the business
process. This health care project will support to customer and provide the awareness

about their health and care. I have continuous access the different equipment sheet,
empirical critical issues and international guide line. Project documentation requires for
managing the features and their specific role towards the hospital management system.
 Reduce the work load- Health and social care project support the entire system and also
reduce the administrative work by centralizing. This project will help for reduce the work
load because it will increase the speed and efficiency in the health care project
management system. Administrative easily feel relax to reduce the complication in the
management process (Noh and et.al., 2018).

reducing the stress level so that people free to increase their income and improve the economic
condition of country.

service increases readily all over the world. This program involves dental care, physical therapy,
medical care etc. All the facilities provided by the hospital and organization with the help of
health and social care project. This is the best way to take good care of the social communities.

Medical health service immediately after disaster- this project will help for identify the disaster
place and providing the medical treatment in different areas and places. Medical services
requires for providing the best treatment to the people (Procter, 2017).
Health care professionals training- I place the under the organization to provide the best
treatment to the patient. It also includes different things such as surgeon will give an idea
regarding the operations and teaching in the hospitals. There are different types of training
equipment available which requires for identifying behind the health issues.
