Reflective Essay: NHS Home Care, Patient Interactions, and Ethics

INTRODUCTION
Home care services and facilities provided by the NHS organisation to their patients who
are not physical capable. Thus, I am a nurse or a care taker of NHS group therefore, my major
responsibility is to take care of patients and provide health care services to them in an effective
and efficient manner (Swayne, Duncan and Ginter, 2012). This report based on my experience
hence, how I will have to be follows NHS values, NHS constitution and NMC code of conduct
while working for patients. Basically, NHS constitution we can be known as a important concept
in which represent rights and powers of all patients. As per this, all NHS bodies whereas private,
public and third sector organisation should provide better services and facilities to patients.
Moreover, in this report, I will discuss about my actual reflective observation regarding patients
as well as which key professional and reflective practices/ issues I will be faced in NHS. On the
other hand, it will also study about Gibbs reflective model and 6Cs in which will be elaborated
my feeling, evaluation and analysis of my experience. At last, I will explain about legal
considerations and legislations which I will be followed while working within the NHS
(Luxford, Safran and Delbanco, 2011).
I am a care taker or a nurse of NHS thus I have some major responsibilities which I have
to be effectively performed within organisation and provide better health care services to
patients. In this order, there are explaining my past observation or experience such as follows:
 I have to systematically observe and gather actual information about his daily routine
work from his family members
 I have to prepare a time schedule, diet chart and medicine chart so that I can provide him
proper services and facilities in a systematic manner.
 Should try to understand his health related problems and issues
 Make a routine for his exercise, yoga and other things (Centers for Medicare & Medicaid
Services (CMS), 2011).
 Prepare a effective chart in which have to be mentioned routine check up and test.
Apart from this, being the nurse in NHS, I have to effectively take care of patients so that
they can recover fast from their disease in an impressive manner. For example: Mr. John is a
Dementia patient of NHS who have short term memory problems due to this he does not
remained any things and information. Due to this, my responsibility is that I have to take care of

him and provide better health care services in work place of NHS along with performing his
daily routine activities such as brushing, bathing, cloth changing , breakfast, lunch, dinner and so
more. Additionally, I have to give emotional and psychological support to him like one of his
family members (Kongstvedt, 2012). Apart from this, I has been faced some issue while
providing health care services and at the time of bathing and communicating with Mr. John such
as:
 Safety and security: While I am working as a nurse at NHS thus Mr. John, I faced
several issues and problems like I felt unsecured while taking care of him because he is
Dementia patient. Moreover, sometimes, may be creates some problems and issues for
mine so that which may be harmful for me.
 Overtime: At work place of Mr. John I had done more works and overtime of my duty
due to this, I cannot spend my personal life as well as I cannot take care of my self. Thus,
it is hard to do work for Dementia patient.
 Environment: In this last, surrounding at work place is also affects on me as well as on
my working capabilities and system. Thus, in which included living style, behaviour, way
of talking and so more (Fisher and Shortell, 2010).
Therefore, I will be defining my professionalism issues in nursing under below:
 Observing and recording all information and behaviour of patients
 Coordinating with physician and other professionals of NHS for developing and creating
customized care plan
 To provide emotional and psychological support to patients
 Effectively diagnosing the disease of patient's symptoms
 Preparing the effective report of patients to maintain medical history, monitoring
changes, routine check-up information and so more (Purnell, 2012 ).
On the other hand, as a care taker of Mr. John, I have my values and ethics as a nurse and
thus, I have to do my work and perform duties as per the NHS Constitution in a fair way like:
 I should have ethical knowledge as a nurse like I have to understand major functions and
solutions to manage and control the situations
 Should take care of practical care due to which I have to always be ready to give
appropriate answer of “What can I do”?

 Ethics is the best source to give appropriate care of patients towards their disease.
Moreover, all my observation and reflective practices are liked with ethics of nurse as
well as 6Cs of nursing group. In the context of 6Cs in which included some important terms such
as care, compassion, competence, communication, courage and commitment.
Apart from this, every business firm has some values and ethics which have to be
effectively follows by every employees or staff members. Thus, there are elaborating values of
NHS constitution in a effective and efficient manner such as follows:
 Should working together for patient
 Develop respect and dignity
 Commitment and maintain the quality of care
 Maintain and prepare the records of all patients(Pleis,Ward and Lucas, 2010).
Therefore, I am going to explaining some NMC- code of conduct such as follows:
 Solemnly pledge my self to dedicate my life and my work to humanity.
 I will manage the utmost respect for persons life rights from time of conception according
to the legislations of the country
 I will not allow consideration of sex, age, religion, ethics, politics to intervene among my
duty and my patient.
 I will be the consider my professional responsibilities with conscience and dignity.
 I will effectively respect and maintain secrets of my patients.
On the other words, there I will be explaining the Gibbs reflective model which can be
adopt and implemented by the NHS. With the help of this model, can effectively analyse and
evaluate the self practices and observation in systematic way ( Tinetti, Fried and Boyd, 2012).
As I am a care taker of NHS thus I am caring of Mr. John because he is a Dementia
patient due to this he cannot remained any thing and communicate. Therefore, while I am
working and take caring of him in a effective and efficient manner, I observed it is not easy to

take care Dementia patient at work place of NHS. Additionally, it is required to be take care of
him in 24 hours his daily routine activities (Epstein and et. al., 2010). In the other hand, as a
nurse of a Dementia patient I have to perform my responsibilities and duties in a effective and
efficient manner such as: I have to maintain records and prepare a report of patient in which have
to be mention all over necessary information about Mr. John such as medicines, check ups, daily
routine, weight, diet and so more.
 I have to provide emotional and psychological support to Dementia patient so that he can
recover by their problems and issues in a effective way (Pollack and Armstrong, 2011).
 Should systematically follow and work according to values and ethics of a nurse.
In the other hand, as a event we can consider Mr. John because he is Dementia patient of
NHS. Thus, he cannot remind anything and cannot communicate in normal way. One of the
major reason behind has been chosen this event because I want to share my experience and
practical knowledge regarding him as well as his diseases. Additionally, I have been faced
several issues and problems while given bath and communicate with him. Due to this, I can
easily share my reflective observation and information about this event (Shortell, Casalino and
Fisher, 2010). Therefore, while I am working for Mr. John so has been face several issues and
problems. Thus, I have various and appropriate ways and techniques so that can effectively
handle him and provide proper health care services and facilities in a systematic way.
In this situation included, me, my other staff members and Mr. John. Therefore, I am
some rights and powers of patients as per the NHS Constitution effectively full fill by the NHS
such as:
 Right to take medical treatment and advice from the professional or doctors
 Mr. John has right to get proper information and records about his health condition
 He has power to accept and refuse the treatments and procedure which provided by my
self (Poksinska, 2010).
 Mr. John have right to maintain some privacy in his life while taking care from my side.
 In the last he has right to complaint to NHS regarding working system of mine.
Self reflection is one of the important and effective way of increasing knowledge and
systematically analysing the observation about a particular situation. Apart from this, I am a care

taker of Mr. John. Thus, I can be used and adopt appropriate reflective model for effectively
analysing and increasing knowledge from my experience. Apart from this, there are basically
three type reflective models such as Gibbs, Johns and Driscoll. Hence, Gibbs reflective model I
has been chosen for evaluating and examining my experience in a impressive manner. In order to
this, one of the major reason behind have been chosen this model for analysing my self reflective
practices in a systematic way. In this model included six major stages which I can be adopt in the
my experience and reflective practices. Description: In the first stage of this model have to be effectively describe the situation
and what happened with me while I was giving bath and communicating with Mr. John at
work place of NHS. Feelings: In the next stage, what I was thinking and feeling towards my that situation and
for Dementia patient (Blackwell, Lucas and Clarke, 2014). Evaluation: Next, I have to be analyse and identify the what was good and bad situations
for me when I was working for Mr. John. Analysis: In which how to handle difficult situations such as bathing and communicating
with Mr. John as well as what sense can I made of the conditions in a effective manner. Conclusion: This is the last of this model is what else and other things could I have done
in my past while I was caring of Dementia patient.
 Action plan: This the last stage of Gibbs model so that in this stage If I am feel it arose
again then what I would like to do as well as how I will be handle ( Evans and et. al.,
2012).

6Cs is one of the major concepts because in which included 6 important terms with the
help of them a nursing staff can effectively deliver excellent health care services and facilities in
a effective and efficient manner. Thus, as per my experience all values are important for patients
not any one more necessary than each others. In the other hand, as I am a care taker of NHS thus
I was adopting and using 6cs concept when I was working for Dementia patient and provided to
him health care services. Now I am going to explain 6Cs in the context of reflective experience
such as follows:

 Care: It is one of the important term for every health care organisation that effectively
deliver by staff members of NHS to all patients so that they can get support as well as
feel better while they are suffering by any diseases. Apart from this, when I was working
for Dementia patient at work place of NHS I had provided proper care and emotionally
support to him so that he can significantly recover from Dementia patient. Additionally,
as per my experience care is our work which have to be provide to all patients. So they
can become capable to face and bear health issues.(Greenrod and et. al., 2014).
 Compassion: This concept helps to define how care and emotional support is given to the
patients within the organisation. Basically, it is based on empathy, respect and dignity. In
the other hand, I was using this concept for Mr. John because he need mercy from my
side.
 Competence: In the context of this concept of 6Cs, nursing staff should have some
ability to understand and ensure about patient's health care needs, medicines and so more
to provide appropriate information and treatments to them but that should be based on
research and evidence. I was a nurse of NHS thus I was provided health care services at
work place to Mr. John. So as per my reflective observation I was deliver proper
treatments and medicines as per the time chart as well a in a systematic manner (Dever,
Fitzgerald and et. al., 2010).

 Communication: It is one of the major and important term in 6Cs concept with the help
of this process patients and staff members can effectively interact and communicate with
each others. Also they can share any kind of health related issues and problems in a
systematic way. In the other hand, Mr. John was Dementia patient due to this he cannot
speak and walk. I was a nurse and I had several duties and responsibilities which I had
effectively performed. As per my observation I was communicate and understand patient
needs and problems through the face experiences, eye contacts, gesture, movements and
so more. Hence, at that time Mr. John using non-verbal communication process because
he is not capable to speak with my self.
 Courage: In this concept of 6Cs, we have to motivate towards our works and tasks due
motivation and encouragement is the necessary term in life without encouragement a
person cannot get success. Therefore, according to my experience I had increased to my
motivation level towards my work as well as I was decided I have to resolved Dementia
problem of Mr. John. Additionally, I also had courage to patient so that he can decide I
have to be fine and resolve my health related problems.
 Commitment: It is the last and important term of 6Cs, hence as per this I took
commitment with NHS and patient for the improve care and resolve the health related
problems and issues in a effective and efficient manner. Thus, while I was providing
health care services and facilities to Mr. John I had commit with NHS professional and
Mr. John I will solved Dementia problem. Apart from this, I used and adopt this concept
for identified and analysed my responsibilities and values which I have to be performed
within the health care service organisation. So that I can systematically provide better and
effective services and facilities to all patients towards their diseases in a Impressive way
(Shah, Goldman and Fisseha, 2011).
As I am a nurse or a care taker at NHS thus I was providing health care services and
facilities to Dementia patient. So I had some major responsibilities and duties in the professional
way which delegated by NHS constitution , which I had effectively performed. Therefore, now I
am going to explain actual meaning of professionalism for a nurse as well as how I had
performed such as follows:
 Non-biased care for patients: In the health care organisation staff members and nurses
are see patients of all ages, size, colour, races and background. Thus, as per my

experience all nurses have to be maintain equality and non-biased nature within the
organisation for patients. Due to this, they can get proper satisfaction and become capable
to recover their health related issues.
 Integrity: Another one duty and responsibility of nurse is have to effectively and
systematically maintain the records and reports of all patients in which should be
mentioned medicines, daily routine activities, condition, diet, recovery level and so more.
Due to this, doctors and professional can understand the actual and current condition of
patients.
 Respect for patients: It is not easy to handle and manage patents in a effective manner
within the health care organisation but it is necessary to does not temper in some critical
situations. As per my observation, it is hard to manage and control on Dementia patient
as well as his impressively take care. Hence, I had control and managed my tempering
level in some critical situations (Mackey, Liang and Novotny, 2013).
 Attitude: While nurses or staff members are working with high attitude and ego then it
can be make problems and difficulties for patients as well a they cannot provides proper
and efficient manner. Apart from this, when nurses of the health care organisation
working and talking with softly with patients so that patients can get proper satisfaction
and increase their motivation level to recover their health issues.
 Willingness to help others: As I am nurse of NHS organisation have to be willingness
nature to help others either patients and staff members. Positive and supportive nature of
nurses helpful for the organisation and patients in the health care institutes.
 Honesty: In the context of health care organisation all staff members and nurses must be
honest towards their works as well as their major responsibilities. Due to this, they have
to be perform all duties in a effective and efficient manner.
 Discipline: In the last have to maintain discipline and decorum of the health care
organisation so that all over activities and health care services and facilities
systematically provided to the patients so that they can get satisfaction towards their
needs.
In the context of this concept, in which effectively analyse and identify about the
advocacy. Thus, nurse have some legal rights and powers to take legal action in illegal situations
and condition which affects on patients. Therefore, at this time period, government of the UK has

regulated and enacted some rules and regulation as well as provided some powers and rights to
nurses and staff members so that they can take legal action against patients family members,
health care organisation and so more if they can hindering the services as well as family
members does not take care of patients. Apart from this, as per my experience as a nurse have to
be properly careful towards patient's health as well as should take care action while doing
negative illegal activities and things by family members and health care organisation for patients.
For example: While I was giving health care services to Mr. John at work place of NHS
so that time my responsibility or duty is should be effectively take care of his health and
problems as well as provide to him proper medicines as per the chart. Moreover,if family
members of Mr. John did not care about him as well as done negative things for him. Thus, I
have right to take legal action against them as well as I can sue the case against them. In the
other hand, if NHS did not provide the proper health care services and hinder the facilities for
patients in this situation I also can take legal action against NHS (Preparing a Healthcare
Organization for Change, 2011). In order to this, nurses can works like advocate for their
patients. Due to this, nurses are provided protection and legal security to their patients in a
effective manner. Apart from this, I has been followed my major roles and responsibilities which
delegated by NHS constitution such as follows:
 As per the NHS constitution I have been provided comprehensive health care services to
patient.
 I have been given NHS services on the basis of clinical need not on patient's capabilities
and ability to pay
 As I am a nurse, I have bees aspired the better and highest standards of excellence and
professionalism.
According to reflective observation government of the UK regulated and enacted some
legal rules and regulations which have to be systematically followed by all staff members and
professionals of health care organisation. So that they can provides better and effective health
care treatments to patients towards their diseases. Due to this, they can get proper satisfaction
towards their health care problems and issues in a effective and efficient manner. Apart from
this, legal considerations is a important concept which helps to manage and reduce illegal
activities within the health care organisation. Legal considerations helps to define, it is the most
important element for the effectively formulation of a legal contract. Apart from this, there are

regulated and enacted some legal rules and legations which have to be effectively followed by
nurses and staff members with in the health care organisation. In the other hand, as I am a nurse
of NHS thus, it is my duty and responsibility is to follows all legal rules and regulation while I
am working within the organisation as well as for patients in a systematic manner. Apart from
this, legal considerations helps to manage and control and reduce illegal activities and things
from organisation. Therefore, as a I was providing health care services and facilities to Mr. John
at work place of NHS so that I have to perform my duties and follows legal consideration in a
systematic way such as follows:
 Safe lifting: Mr. John was a Dementia patient who did not speak and remind so that I
was providing health care services and facilities at work place of NHS. In this situation
my legal duty is have to be safely lifting to him for changing their their clothes, for giving
lunch and dinner, bathing and other works because he were does not capable to
effectively done all these daily routine activities in a systematic manner.
 Person care: In the context of this, my duty is to be effectively care of Mr. John as well
as provide him some emotional and psychological support so that he can motivate
towards recover his health problems in a effective and efficient manner. Personal care an
support is the best aspect because with the help of this effectively increase the capabilities
and bear powers.
CONCLUSION
From the above mentioned report it has been concluded that NHS is a publicly funded
health care organisation of UK and I was a nurse in this health care institute. Therefore, there I
has been elaborated about major duties and responsibilities of nurses while they are working in
health care organisation. Apart from this, in this report I was providing health care services and
facilities to Mr. John because he is Dementia patient due to this he did not speak and remind
anything. Moreover, I has been used Gibbs reflective model for analysing and evaluating my
observation in a effective and efficient style. In the last has been define some legal consideration
which had to be followed by all over staff members of health care institutes.
