Reporting-
In the given case scenario, Joe is a 67 year old man that lives near the rural area on NSW
South coast. He runs his father business as an oyster farm and has worked there since he left
childhood in the age of 15. He is overweighted and suffers from disease as gout, osteoarthritis,
migraines and hypertension. From the last some years, Joe son Max has taken the most of the
responsibilities of the farm. Joe presently look over the responsibilities as sales and bookkeeping.
From the time when Joe stepped back from the maintenance work he felt that his physical
conditions gets more worsen. He always feel restless, starts becoming breathless after walking up
few stairs and find it increasingly typical to move trays and boxes of oysters around.
The one of the main issues that has been identified in the given case scenario is that Joe is
already overweighted and from last few year he start to look over accounts related work only. It
is not work in which person need to physically workout. Due to it, he became more fatty and felt
tired every time. I also felt that this situation has made his physical condition more worsen. It
also has been noticed that he belongs to town and after retirement of his regular General
practitioner, Joe became unable to find better GP in the areas from he belongs. Currently, he has
to drive around 40 minutes to visit medical centre to have new script in Batsmans Bay.
He was not able to find good GP in the area and this situation made him to not visit the
same more than once. I felt that ineffective facilities of medical is one of the major issue to deal
with the his physical conditions. I have also noted that his diet was also not good according to his
age. Every day he use to eat seafood such as fish, chips etc. and drinks 3-4 glass of wine each
evening with dinner. I must say that Ineffective nutrition and improper diet has made his physical
situation more deteriorate. It is important to appoint the General practitioner in the town so that
residents can provide better treatments. Effective medical facilities within town area can also be
assistive in term to aware people about the well being and care.
I must say that he need to consult with one medical practitioner about his medical
condition on regular basis. He must took his advice and need to properly follow the diet
recommended by GP. He must avoid sitting work as he is already overweighted. He must intake
the Alcoholic beverages once in a while and also do some sort of exercise daily to improve his
physical conditions.

I have acknowledge this situation earlier in my professional experience. The ineffective
care of medical facilities has harsh impact on the health of individuals. Joe was overweighted
and his physical conditions get more worsen day by day. In my point of view, Joe needs to take
the regular monthly check-up from medical professionals. Once, I have encountered a patients
with the similar situation. I have analysed that improper diet and lack of medical facilities in the
areas he lives made his situation more worsen. This both the situation was similar and both were
having the great impact on their health. In the previous case, the patients was facing the high
level of obesity with the chronic situation as Heart. He was also facing the similar situation of
not finding any better General practitioner in the area he belongs, this situation has made his
physical condition more worsen. In order to get overcome it, this is crucial to appoint one
medical practitioner in the area and this could helps to get overcome from this kind of the
situation in more effective and efficient manner. Additionally, the better and well advanced care
facilities can aids to maintain the health status of the individual (Cantor and Thorpe 2018) .
In order to deal with the situation effectively, there is need to have proper skill within
medical practitioner so that better and effective treatment can be given to patients. I have good
communication skill and that will help me to make interaction with the partitioner and can also
assist me to do the things properly. Thus, good communication skill can lead me to do the each
thing more effectively. In my medical experience, I have diagnosed each situation very
effectively and this lead me to contribute the things in more nicely manner. Thus, the people
must aware about the well being and about the nutrition intake. I must recommend that patients
must have taken regular exercise and this will help him to be more physically healthy. I must
recommend that I have analyse the prior condition with help of maintaining the online
communication with patients. I used to make interaction within each 7 days. I used to visit him in
each 15 days and took the necessary check-up. I recommended him nutrition chart about the
daily intakes and this kind of the things has helped me to get overcome with his physical
conditions. In that time, I have continually monitored his physical requirements and provide
assistance at time he required. This situation has helped me to get overcome with issue and the
proper care has been taken to maintain the health status of that individual.

another reason was that his intake was not proper, as he use to eat seafood, chips etc. This
situation made his physical condition more worsen.

There are number of components that come together and that great affect on the health of
individuals and communities. As per the view of Marmot, (2018) stated that the the number of
the social determinants of the health are as education, stigma, racism and this all are underlying
contributor factors of health inequalities. Henceforth, the factors as environment, income,
education level and genetics affects the relationship with friends and family. Thus, this kind of
condition highly influence the health status of the individuals. Therefore, there are some
common determinants of the health as are-
The social and economic environment
The physical environment
The person individual behaviour and characteristics.
Additionally, the range of the personal, social, economic and environment components
may influence the health status of the individuals. There are interrelationship among these
components that determine the population and individual health. However, these kind of
intervention that target the multiple determinants of the health of an individual (Bambra Gibson
and Petticrew, 2010). Henceforth, the some of the determinant are defined in the following
manner as are-
Health- Socio-economic differences in the health are persistent, widespread and large across the
different societies and it has diverse range of the health outcomes. However, the common
indicators are as income, occupational status and educational attainment. Hence, this potential
factors in relation to health behaviour affects the health of individual in the negative aspect.

Employment- It plays the big role in health as this offers financial stability, promote
independence and this all are the major factor against the physical and mental health problems.
Ineffective employment status are major contributor to ineffective health conditions of the
individuals.
Educational attainment- Low education level of the individual are mainly linked to poor health
outcomes of the individuals (Hilz, Conner and Schüz, 2019). The more stress situation and lower
self confidence impacts the health of the individual in harsh manner. Thus, the people must have
equal education opportunities and all needs to be aware about the well being of the individuals.
Social support networks- The greater support from the families, communities and friends is
linked to the better heath. Additionally, it can be stated that the culture, customs and tradition all
has significant affect on the health of individual. This has huge impact on the health of
individuals.
Income- The measurement of the income is more complex than accessing to the educational
attainment. Henceforth, the income level of people decides the heath outcome of individuals.
As per the view of Northwood, Ploeg and Sherifali, (2018) stated that social and
structural of the health define environmental conditions both physically and social that highly
affecting the health outcome of individual. Additionally, the influence of culture affects the
perceptions of health, illness and death of individuals. Furthermore, this can be stated that social
and structural determinants of the health highly impacts the health outcomes such as biological
and individual level factors. Thus, cultural competence is highly for the bringing improvement in
patients health.

In the given case scenario, Joe has faced the health inequalities due to improper education
and lack of social support networks. This has impact the health of the individual in the crucial
aspect. If this kind of situation occurs in the future, the individual must provide the proper care
facilities and well being to the individual. Thus, all individual has right to know about the well

In future, I would promote education to all individual so this kind of situation do not

In order to get overcome from this kind of the situation can be possible with the help of
undertaking the centring care on people needs and preference. It can be stated that informed and
engaged patients must invest in their own health care as well as to take steps to bring
improvement in the broader health care system and this is most effective and crucial to learning
system. Under this, patients bring unique and crucial perspective on their own care on the
experience of the health care conditions. Thus, patients, families, caregivers and other public
needs to get engaged in the care and within the patterns of improvement. In terms to deliver the
effective healthcare, the better teamwork can immediately and positively impacts over the
patients safety and outcomes. So, it can be stated that the interdisciplinary teamwork plays the
crucial role for delivering better and improved health care to the patients.
Henceforth, this can be stated that the effective teamwork is one of the globally recognized as
one of essential tool for constructing more effective and patient centred health care delivery
system. With help of identification of the best practices and better support of all medical
professionals may assist to carry out effective working. Additionally, the lack of patients focus
on the health care system is also a evident in patients transition between the care settings. This
kind of the situation can be handled with help of patients centred care and to take steps to
undertake personal health maintenance facilities.
