placement in the first year, I have encountered a patient with hypertension, who was quite
agitated because of me as I left her unattended while attending others and the medication I have
provided patient caused her discomfort such as he was experiencing difficulties in breathing.
After thorough observation, I have understood that there was some medication and
documentation error which made him uncomfortable and he became agitated. I reviewed the
ISBAR of the patient and gained the understanding that I documented diazepam instead of
Diltiazem which made the patient agitated and uncomfortable. I reviewed his ISBAR again and
communicated my supervisor to look after the medication errors I have done and apologized to
him and requested for appropriate medication in order to treat his hypertension. I ensured that he
will receive a quality of care supported him through effective communications in order to
improve his mood. From this incident, I have gathered the understanding that in order to provide
evidence-based care to the patients, it is crucial to gather experience and knowledge in the
clinical setting which will facilitate critical thinking.

In the clinical setting, use of effective interpersonal skills, negotiation and conflict
resolutions while working in collaboration with the team, may reduce stress, promote the
wellbeing of the professionals and improve quality of life as well as clinical practice. While
working as a registered nurse, I have gathered the understanding that active communications.
Flexibility, patience, negotiations and conflict resolutions are the most crucial attributes of a
nurse which facilitate the practice and assist to empower patients. During my clinical placement,
I have encountered an incident where conflict of opinion observed between two of my coworkers
regarding the effective interventions for a mental health patient. After detailed observation, I
have observed that both of the coworkers were right in their positions. While one suggested to
involve family members of the patients against the will of the patient as it would facilitate faster
recovery, another workers suggested that breaching autonomy of the patient is an ethical issue.
In order to resolve this conflict, I communicated with my supervisor and communicated with
both the coworkers. I have suggested to follow both the interventions through negotiations such
as involve family members indirectly in the decision making. After implementing this combined
intervention and resolving this conflict, I have evaluated that these skills improve better decision
making, promotion of collaborative practice and facilitate communication between team
members.

Effective written and verbal communication:
Effective written and verbal communication on the clinical setting is imperative to
provide the best care to the patients around the globe. Effective written and verbal
communications help en enhances the professionals nursing practice and improves the
relationship with various stakeholders by reducing the high prevalence of medication error and

make a difference in positive patient outcome. While I was in my clinical practice, I was
allocated with the responsibility to check an aboriginal patient with severe lower abdominal pain,
indicated a kidney problem. After communicating with patients, I did not understand what she
was saying due to the difference in language. However, from the nonverbal communication, I
had gathered the understanding that she was agitated because of language barriers she was
experiencing while communicating with me and other health professionals. After reviewing her
ISBAR report I have gathered the understanding that there was documentation error such as I
documented wrong information regarding the medical history, due to lack of language
proficiency. Consequently, she was offended and agitated due to the services she had received.
In order to resolve this issue, I communicated with my supervisor in order to recruit a translator
who will translate the conversations into English for facilitating the communications and
effective documentation. After recruitment of translator, she was able to communicate with the
translator in her native language which reduced the psychological distress she was experiencing
and facilitate the diagnosis and documentation. From this incident, I gathered the understanding
that effective written and verbal communications facilitate the medical procedure, improves the
nurse and patient relationship and promote safety.
Question 4:
In the clinical setting, quality improvement is one of the crucial factors which ensure
high quality of care to attack patients and retain the nurses, improve experiences of care of
patients. Risk assessment is one of the crucial parts of the quality improvement initiatives. In the
clinical placement, I have evaluated that many of the patients became malnourished due to
inadequate nourishment was given to them which increased the morbidity rate in the hospital and
reduced the revenue in the hospital. In that situation, in order to improve the quality, I have

each patient I collaborated with my supervisors in order to implement a quality improvement
initiative. After discussion with other coworkers, I proposed to conduct through screening of
patients during admission and create color-coded papers for the patients who require extra care.
After implementation, we have evaluated the outcome and found out that it improved the
nutritional status of the patients, improved the quality of the care, reduce the high morbidity and
facilities the faster recovery in the patients and improve the professional development and
practice of the health professionals.
