Hello, I hope this report finds you well. I am a support worker at the disability organization in
which you are a manager and I am writing to you regarding Kw’s situation on 28/02/2020. I
noticed that she was very stressed and upset since I reported for my shift and she kept using the
terms “please” and “I was wondering” while at the same time laughing as I talked to her. She
additionally claimed that she was at the risk of losing St Michael that she had been using on
behalf of her dog unless she became more polite. While talking about her dog, she laughed as she
said: “Sapana Mya was wondering if you could please give her food and she is also wondering if
you please make more food for tomorrow”. She was wondering why she was so worried about
being too polite and further stated that some of the staff at the organization claimed that she was
being too polite to them and they no longer wanted her to be that excessively polite but instead
try to be more formal. Even though I had not mentioned anything to her about her politeness, she
kept asking me to ask Pauline if she thought she was polite as stated by the other staff at the
organization. My efforts to divert from the topic by saying that I did not want to talk about other
staff did not bear fruits most of the time because she kept going back to the same issue of
politeness.
I assisted her with regular duties this evening including cooking, cleaning, medication
administration, and showering. I also encouraged her to do crafting and keep herself busy in an
attempt to divert her attention. She seemed very distressed when she went to bed at 10 pm as she
kept repositioning and talking to herself. She took her phone with her to the bed and started
typing a very long email while talking to her dog Mya. She could not stop typing the email
despite my attempts to convince her to stop and sleep. She claimed that typing that email could

make her feel better. I noticed that she fell asleep at around 1 am but she was up and talking to
herself again at 3 am before sleeping at 3.45 am.
She had mentioned earlier that she had a plan but she did not go into the details of the plan. This
left me worried about what she could be intending to do therefore I kept monitoring her. It was
therefore impossible for me to sleep at night and I went to her room four times to check on her.
However, at around 4.30 am I fell asleep on the office chair and woke up at 7 am. I found her in
the dining room talking to Mya. She was restless and pacing so I helped her with showering and
dressing before I made her breakfast. I checked her BSL and administered her medications and
insulin. I also assisted with feeding her dog Mya before handing her over to change over staff.
When I came back for my evening shift on 29/02/2020, I noticed that she was again very upset
and she cried for a while. She was complaining about the treatment she was receiving from other
staff and the management. She talked to her friends Sharon and Sarah about Abby did and what
Pauline said about her issues. Her friends have been a source of strength for her. We went to buy
groceries and I prepared dinner for her. I assisted her with cleaning and showering before
administering her medications. She took a glass of milk before going to bed. She talked about
feeling suicidal and asked me to take care of Mya. She even asked me to write some notes about
her suicidal thoughts and in her notebook. She said that these notes were meant for her support
coordinator to understand her true feelings. I wanted to call Pauline but she asked me not to and
went further to promise me that she would not do anything crazy. I, however, had to inform the
management because I was worried that she could harm herself. She went to bed at 9.35 pm and
fell asleep at 10.20 pm before waking up at 12.30 am to go to the washroom. After repeatedly
adjusting her position she finally fell asleep and did not wake up until 8.35 am. I assisted her

insulin. I handed her over to change over the staff at 9.45 am.
